BackgroundWith the increase in the number of elderly population,the health of the elderly population has become a hot issue,and in many elderly health effects of chronic diseases,high blood pressure is the most important and serious diseases.Hypertension complications and more serious,including heart,brain,kidney,fundus and other vascular diseases,the elderly patients with hypertension,quality of life and life caused serious damage.Domestic and international studies have shown that continuous care can bring benefits to the rehabilitation of patients to meet the needs of patients,and ultimately to improve the quality of life of patients.There are many chronic diseases in our country,such as diabetes,chronic kidney disease,chronic obstructive pulmonary disease,cancer and so on.However,there is little practice in elderly patients with hypertension.It is important to study the effect of continual nursing care on the quality of life of elderly hypertensive patients.ObjectiveThe perpurse was to investigate the impact of continual nursing care on the quality of life of elderly patients with hypertension in order to improve the quality of life of elderly hypertensive patients.Methods1.Study subjectsA total of 300 patients aged 60 years and over were enrolled in this study from May 2015 to April 2016.The patients were discharged at the time of hospitalization,The patients were divided into the control group and the observation group,each group of 150 cases.2.Research methods(1)Continuing nursing content and method Two groups of patients in the hospital are undergoing routine care of hypertension,including hospitalization during the health education and discharge guidance.After discharge,the control group no longer received continuous care,observation group discharged within 3 months of continuous care.(1)continuity of care content.Content includes: diet,rest,exercise,mood,drug knowledge,smoking cessation limit alcohol,weight control and so on.continuity of care methods.(2)Weekly telephone follow-up;every 2 weeks for 1 out-patient follow-up or the researchers to the patient's home for family follow-up;use of WeChat group at any time to patients or their families to give care content supervision and problem solving.(2)Observe the main observation indicators include:(1)Quality of Life The quality of life was assessed by SF-36 quality of life,the main contents include: physical function,physiological function,physical pain,general health status,social function,emotional function,mental health 8 dimensions.The Cronbachs' a coefficient was 0.865;(2)Ability of daily life The Barthel index was used to evaluate the daily living ability,including feeding,transfer,modification,toileting,bathing,walking,walking up and down stairs,wearing undress,stool control,Urine control 10 items.;(3)Blood pressure values,including blood pressure during hospitalization,the patient three times a week at home self-test blood pressure values,the researchers every two weeks of home visits to patients or outpatient follow-up blood pressure values.Results1.The quality of life of the two groups of patients There was no significant difference in the quality of life between the observation group and the control group at the time of discharge,t = 0.146,P> 0.05.There was significant difference in the quality of life between the observation group and the control group at 3 months t=2.560,P<0.05.There were significant differences in the quality of life of the observation group between 3 months after discharge and discharge,t = 2.022,P <0.05.The quality of life of the three months of discharge in the observation group was significantly lower than that in the discharge group(P <0.05).2.Two groups of patients with daily living conditions There was no significant difference between the observation group and the control group at the time of discharge(?~2 = 0.094,P> 0.05).The difference between the observation group and the control group was statistically significant after 3 months of discharge,?~2= 3.892,P <0.05.3.Blood pressure in the two groups of patients There was no significant difference in systolic and diastolic blood pressure between the two groups at discharge,(t=1.292,t=1.156),P>0.05;In the discharge of 3 months of systolic and diastolic blood pressure,the difference was statistically significant,(t=3.162,t=3.487),P<0.05.4.Influencing factors of quality of life There were significant differences in the scores of life quality of elderly hypertensive patients between the observation group and the control group in terms of age,education level,occupation,monthly income,medical insurance and so on,the difference was significant(P <0.05).ConclusionContinuity nursing care of elderly hypertensive patients after discharge can improve the quality of life and daily living ability of patients and control blood pressure stability.The quality of life in various dimensions of physiological function and social function of elderly patients with hypertension in the impact of quality of life,nurses should consider these two dimensions,to explore effective nursing interventions.
