Research On The Current Situation And Countermeasures Of The Provision Of Continuous Service Of Rural Medical And Health Service Network In China

Starting from 2009,along with the deepening reform of the medical health system,the central and local governments spend a lot of financial support at the construction of county-level hospitals and community-level medical and health institutions.The rural medical and health service system has been significantly strengthened,which has greatly improved the accessibility of medical treatment for rural residents.However,fragmentation of medical service system,disorderly competition between organizations,and fragmented services,has caused the low efficiency of medical service system,higher medical expenses and deterioration of medical environment,always restricts the overall efficacy of the rural three-level"network",hinder the improvement of the utilization efficiency of rural basic medical service,become the shackles of function optimization of rural medical service system.At the same time,with the speeding up of the trend of population aging,the extension of life expectancy,aggravating burden of chronic non-communicable disease,more and more patients need that different levels,different types of medical institutions provide medical services together,and expect to enjoy the integration of health service including prevention,treatment and rehabilitation.The challenges faced by both the supply and demand of medical services require that the health service system must be a continuous and collaborative system.So,pursuit vertical integration of medical resources,achieve cross-sector cooperation,coordinate the balanced development of the medical service delivery system,as a whole to ensure the medical quality,reduce medical costs,provide continuous,the overall medical service effectively,become the inevitable development direction of rural health service network.After summing up the research of domestic and foreign scholars,this paper focuses on the current situation of continuous service provision in the rural three-level medical and health service network,mainly including the continuity of the provision of health services by different service windows,the continuity of health services provided by different types of services,the continuity of the provision of health services by different levels of institutions,and the continuity of the provision of health services by different departments and organizations.The aim is to find out the discontinuity and fragmentation of rural health services and to explore the causes and propose corresponding countermeasures.The present study finds that the fragmentation of rural health service system and the fragmentation of service delivery are mainly manifested in:firstly,the division between public health and medical service;secondly,prevention,diagnosis and treatment,health promotion,health education and other phase are separated;thirdly,there is a lack of effective cohesion between different levels of medical institutions;fourthly,the sharing and transmission of medical information has been broken;fifthly,there is a lack of coordination and interaction among multiple providers.On this basis,this paper explores the real causes of the problems from the aspects of resources,organization and administration by the macro model of health system.In view of the problems existing in the continuous service of rural health service network,this paper proposes the following countermeasures and suggestions based on the actual national conditions of China:firstly,adhere to the government’s dominant position and improve relevant supporting policies;secondly,increase medical and health input and integrate medical and health resources;thirdly,improve the quality of health personnel and strengthen the capacity building at the grassroots level;fourthly,promote cooperation and interaction among institutions and improve the mechanism for sharing benefits;fifthly,improve the hierarchical medical system and promote orderly medical treatment;sixth,improve the medical information platform and improve the information sharing mechanism;seventh,improve the supervision and control mechanism and improve the system of assessment and incentive.The paper hopes to provide certain reference value for promoting the rational allocation of health resources,taking advantage of synergies and overall,meeting all kinds of residents,health needs,providing coordination and overall health services.
