Objective: The rural health service network in Wuzhishan City is targeted in this study. By investigating in the present situation, the utilization of the network by the local people, and the equity of the health service network, it aims at the continuous development of the cooperative medical and health care system in the poor rural regions in Hainan Province. It is expected to provide with scientific guidance and information, as well as theoretical directions for rational allocation and distribution of such a network in the rural areas in Hainan.Method: The supplier of the health service includes all the hospitals at the township level in Wuzhishan City. The survey is carried out by collecting the available data and an general investigation in the present conditions of the hospitals by means of a questionnaire The demander is composed of the people in the 7 townships which are divided into 3 groups according to their economic conditions. The final objects of 65 households from each village in each township of each of the 3 groups are randomized respectively for this survey.Results: 1. The allocation of funds for each township health center varies from its farming population with the highest average in Chongshan Township as 44.66 yuan per capita and the lowest in Maoyang Township as 15.58 yuan pcr capita ( P<0.001). 2. The health professionals are unequally located in the townships with 46% in Chongshan Township and only 4.6% in the most remote Township of Maodao. (P<0.001), In the age structure of the professionals, those who are 45 years of age and above take the dominant part of 36.78% while those who are under 25 years old are mainly located at Chongshan Township (P<0.001). In regard to professional structure, 44.4% of the doctors and 48.5% of the nurses are located at Chongshan Township and there arc only 3 doctors (6.7%) and I nurse (3%) working at the remote Maodao township. 3. The equipment and apparatus for the township health centers vary greatly from place to place. The most common equipment and apparatus are sphygmomanometers, stethoscopes and thermometers. The only set of ultrasonic Type B scanner is at Chongshan Township Hospital. 4. The number of sickbeds, medical and heallii professionals, doctors and nurses for each thousand of farming population in Wuzhishan City is 1.454, 2.161, 0.861, 0.631 respectively. 5. Medical and health service carried out in the township health centers in 2003 are as follows: 56434 outpatient visits, 14980 cases vaccinated, 409 inpatients with only 5.9% of the bed utilization rale. 6. There are altogether 74 country doctors, 27 medical assistants and 108 midwives in Wuzhishan City with an average of 1.98 country doctors and/or medical assistants per village, 1.932 doctors and/or medical assistants, 2.066 midwives for every thousand of the rural population 7.
