Objective:(1)To investigate the situation of self-care angency and main complications of nasopharyngeal carcinoma patients after discharge;(2)To analyze the main influence factors of self-care angency;(3)To explore the effect of continuing care based mHealth App for nasopharyngeal carcinoma patients after discharge.Methods : From September 2016 to December 2016 in the first affiliated hospital of guangxi medical university radiotherapy department,156 outpatients with nasopharyngeal carcinoma were invesigated,using the general information questionnaire,the complications questionnaire after radiotherapy and Exercise of Self-Care Agency(ESEA),General Self-Efficacy Scale(GSES),Self-rating Depression Scale(SDS).Using the convenience sampling method to select from January 2017 to August 2017 in the first affiliated hospital of guangxi medical university radiotherapy department after treatment with radiation and chemotherapy of 105 cases of patients with nasopharyngeal carcinoma,according to the discharge time sequence,from January 2017 to April 2017 discharged from hospital after radiotherapy department 52 cases as the control group,from May 2017 to August 2017 discharged 53 cases as the experimental group.The control group given conventional discharge guidance,and the experimental group given based on mHealth App continuing care.Use the complications questionnaire after radiotherapy ?the Exercise of Self-Care Agency ? General Self-Efficacy Scale ? Self-rating Depression Scale ?Quality of Life Questionnaire-core to evaluate.Results:1.The self-care agency of patients with nasopharyngeal carcinoma: the average score was(107.38±18.56),in the medium level.The dimension score from high to low order was: the responsibility(2.94±0.63),the self concept(2.76±0.66),the health knowledge level(2.35±0.37),the self-care skills(2.33±0.57).2.Nasopharyngeal carcinoma patients discharged from hospital main complications within 6 months were dry mouth,stuffy nose and mouth difficult.3.The influence factors of self-care agency in nasopharyngeal carcinoma patients after discharge:(1)There were significant differences in the self-care agency score among different cultural degree,marital status,occupation,family income main source,the medical expenses bear way,family support(P<0.05).(2)Patients with dry mouth,stuffy nose,mouth difficult self-care agency score and four dimensions are lower than no dry mouth,stuffy nose,mouth difficult patients(P < 0.05).(3)There were significant differences in the self-care agency score and four dimensions between different self-efficacy and depression(P<0.05).(4)Multiple linear regression analysis showed that the cultural degree,marital status,family support,dry mouse,self-efficacy,depression were the main factors that influenced the self-care agency of patients with nasopharyngeal carcinoma,which explained 81.3% of the variation.4.The effect of continuing care based on mHealth App:(1)Comparison of complications after radiotherapy between two groups: there were no statistically significant differences in the severity of dry mouth,stuffy nose,mouth difficult between two groups before intervention(P>0.05);3 months after intervention,there were no statistically significant differences in the severity of dry mouth,stuffy nose,mouth difficult(P>0.05);6 months after intervention,the experimental group were lower than the control group in the severity of the congestion,dry mouth,mouth difficult(P<0.05);the experimental group were lighter than the control group in the severity of dry mouth,stuffy nose,mouth difficult after ntervention,the differences were statistically significant(P<0.05).(2)Comparison of self-care agency between two groups: there were no statistically significant differences in self-care agency score and four dimensions before intervention(P>0.05);the experimental group were highter than the control group in self-care agency score and four dimension scores after 6 months after intervention,the differences were statistically significant(P<0.05).(3)Comparison of self-efficacy between two groups: there were no statistically significant differences in self-efficacy score before intervention(P >0.05);the experimental group was highter than the control group in self-efficacy score after 6 months intervention,the difference was statistically significant(P<0.05).(4)Comparison of quality of life between two groups: there were no statistically significant difference in quality of life before intervention(P > 0.05);the experimental group was highter than the control group in the body,character,emotion,cognitive function and overall health scores,and lower in fatigue,pain,loss of appetite after 6 months intervention,the differences were statistically significant(P <0.05).Conclusion:1.Nasopharyngeal carcinoma patients showed self-care agency was in medium level after discharge,and the health knowledge and self-care skills were poor.2.Nasopharyngeal carcinoma patients discharged from hospital main complications within 6 months were dry mouth,stuffy nose and mouth difficult.3.Cultural degree,marital status,family support,dry mouth,selfefficacy,depression were the main factors that influenced the self-care agency of patients with nasopharyngeal carcinoma.4.Continuing care based on mHealth App can reduce patients' dry mouth,stuffy nose,mouth difficult extent,improve patients' self-care agency,self-efficacy and quality of life and health outcome.
