Objectives:The paper was to provide the continuing care for the elderly patients with type 2 diabetes,to examine the effect of continuing care on self-management,self effect and quality of life,and then to provide scientific basis for hospitals to carry out continuing care in the future.Methods:Convenience sampling was used to select 75 elderly patients with type 2 diabetes in one first-class comprehensive hospital from August to October,2016.The sample was divided into two groups,using coin tossing method.In the study,descriptive statistics,frequency analysis,X2-test,and independent t test were performed to analyze the data using SPSS 17.0.Results:1.Descriptive of Demographic characters and Homogeneity analysis:About 46.7%of the samples aged from 50to 60years old,and half of them were Han Race.Sixty-three percent of the samples complicated with other two or one chronic diseases.There was no statistical difference forthe total score of self-management,self-effect and quality of life between intervention and control group(P>0.05).2.Effect of continuing care on self management:After the continuing care,the total score of self-management in intervention group(25.03±1.99)was significant different from(P<0.01)that in control group(18.44±3.08).3.Effect of continuing care on self effect:After the continuing care,the total score of self-effect in intervention gro(88.55±10.69)was significant different from(P<0.01)that in control group(79.70±7.84).4.Effect of continuing care on quality of life:After the continuing care,the total score of quality of life in intervention group(73.16±5.23)was significant different from(P<0.01)that in control group(90.22±5.42).5.Effect of continuing care on physiological and biochemical indexes:After the continuing care,the scores of FPG,2h PG,HbALc in intervention group and control group was significantly different,while there were no significant difference(P>0.05)between intervention group and control group on the scores of TC(mmol/L),TG(mmol/L),HDL-C(mmol/L),LDL-C(mmol/L),BP(mmHg),and BMI(Kg/m2).Conclusions:1.Continuing care could improve the self-management level among the elderly patients with type 2 diabetes.2.Continuing care could improve the self-effect level among the elderly patients with type 2 diabetes.3.Continuing care could improve the quality of life among the elderly patients with type 2 diabetes.4.Continuing care had significant impact on some of the physiological and biochemical indexes among the elderly patients with type 2 diabetes.
