Objective(1)To know about basic information and the activities of daily living, as well as the status of the demand for community care services in Huaihua City.(2)To analyze the influencing factors of post-stroke patients' care needs.(3)To investigate the intervention measures and effects, improving nursing quality of cerebral apoplexy patients in community.MethodsFrom March 2014 to December 2014,a total of 197 cerebral apoplexy patients from five community health service centers in Huaihua city were enrolled. Applying the general data questionnaire,the investigation scale of stroke patients with community care needs which should be complete by patients, and Barthel index scale that required the researchers to full out when we got answers from the patients. According to the results, we formulated the intervention program in corresponding community rehabilitation nursing.The selected 116 patients got the higher score and ADL < 60 with the same degree of care as the matching condition, and were divided into intervention group and control group. Different nursing interventions were given to patients with post-stroke, the patients in control group received face-to-face lectures for centralized education, and those in intervention group received additional implementation of community rehabilitation nursing intervention program that based on the centralized education. After six months, evaluating the effects of interventions. SPSS17.0 software package was used for the main descriptive analysis, t test, and Chi-square test, as well as variance analysis and multiple linear regression, etc.Results(1)The total score of community care needs of the stroke patients was 60.95±14.18, all dimensions' scores from high to low in turn are rehabilitation professional training needs, psychological nursing care needs, social support, life, health education and self care needs, and complications of care needs.(2)Compared to the patients with different age, culture level, occupation, income, health insurance form, and whether the number and once accepted the professional rehabilitation training score, there were no statistically significant difference(P > 0.05), But the comparison with different nationality, gender, marital status, disease diagnosis, course score, the difference was statistically significant(P < 0.05).(3)There were significantly decreased after six months of community rehabilitation nursing intervention, each dimension scores of intervention group in community nursing demand scale, the difference was statistically significant(P < 0.05).(4)There were significantly difference between two groups, each dimension scores of intervention group in community nursing demand scale, the difference was statistically significant(P < 0.05).(5)There were significantly difference between two groups, the scores of intervention group in Barthel index scale, the difference was statistically significant(P < 0.05).Conclusions(1)The nursing demand of stroke patients in community were the upper level. The main influencing factor are stroke patient's age, nationality, gender, marital status, disease diagnosis, of course, ADL score.(2)The community nursing intervention can induce the care needs of patients and improve life self-care ability.
