ObjectiveTo explore the effect of feeding training combined with ice normal saline oral nursing intervention on the rehabilitation effect of stroke patients with dysphagi.To seek a combined,effective and simple nursing method for the improvement of the deglutition function,nutritional support and reduction of complications of patients with cerebral apoplexy dysphagia.MethodsA total of 128 patients with deglutition disorders who met the inclusion criteria were selected as the study subjects among stroke patients hospitalized in the Neurology Center of a Grade III Class A hospital in Jingzhou City,Hubei Province from December2019 to December 2020.According to admission time sequence using randomized groups,the researchers according to random number table method,choose any Numbers in the table to locate,128 Numbers in turn according to the order listed in the table,the number of extraction separately divided by 2,according to the different remainder 0,1,respectively corresponding to the control group and experimental group,control group(n = 64),the treatment group(n = 64).The control group(n=64)was given routine nursing care for dysphagia in neurology department,including behavior guidance,environment improvement,physical therapy,rehabilitation training(oropharyngeal muscle movement training,tongue and soft palate training),breathing training,arthroplasty training and other psychological nursing care.On the basis of the control group,the experimental group(n=64)was given feeding training + ice saline oral care,including swallowing screening,eating position,food selection,mouthful size,feeding style,swallowing posture,ice saline oral care,nursing education,etc.In this study,SPSS22.0 software was used for data cleaning,statistical description and analysis.The general information and scores of each scale were described and analyzed by means of mean±standard deviation,frequency,percentage,etc.Measurement data in the two groups of data were compared between groups using two independent samples t test,which was expressed as meanstandar±d deviation.The rate or percentage of counting data is expressed by chi-square test or continuous correction chi-square test;Rank sum test was used to compare the grade data,and the difference was statistically significant(P < 0.05).To evaluate the effect of swallowing function recovery,nutritional status,incidence of aspiration pneumonia and quality of life in 2 groups before and after intervention.Results1.General data comparison: two groups of patients in patients with age,gender,marital status,education degree,the main caregivers,caregiver and patient relationship,diagnosis of diseases,occupational status,monthly income,kubota drinking water test swallowing condition,BMI,differences compare no statistical significance(P > 0.05),two groups of patients with baseline is consistent,comparable.2.After intervention,grade I and grade II in the test group accounted for 42.19%and 57.81% of the patients in the low-lying drinking water test.The grading of drinking water test in depression field in control group was grade II,grade III and grade IV,accounting for 7.81%,48.44% and 43.75%.The experimental group was significantly better than the control group,indicating that the recovery effect of drinking water test in the depression field was better in the experimental group than in the control group,and the difference was significant Statistical significance(P<0.05).3.After intervention,the recovery of swallowing function in experimental group was better than that in control group,and the difference was statistically significant(P<0.05).4.After intervention,the nutritional risk value of the experimental group was2.42±0.06,and the nutritional risk value of the control group was 3.97±1.12,the difference was statistically significant(P<0.05);After intervention,the hemoglobin value of test group was 131.50±14.35,and that of control group was 125.56±17.42,the difference was statistically significant(P<0.05).After intervention,the serum protein value of the experimental group was 68.46±5.84,and that of the control group was66.97±4.69,with no statistical significance(P >0.05).After intervention,the total protein value of the experimental group was 39.5±74.30,and that of the control group was 36.92±4.30,the difference was statistically signif icant(P<0.05).5.After intervention,the incidence of pulmonary infection in the two groups was(0%)and(1.56%),respectively,with no statistical significance(P>0.05);After intervention,the incidence of aspiration in the two groups was 3.12% and 25%,respectively,and the difference was statistically significant(P<0.05).Ingestion training combined with ice normal saline oral nursing intervention method has a positive effect on the recovery of swallowing function and the prevention of malnutrition in stroke patients with dysphagia.Ingestion training combined with ice normal saline oral nursing can improve the strength of the patient’s swallowing muscle,reduce aspiration and prevent the incidence of aspiration pneumonia.
