Objective The aim of this article is to analyze the direct and indirect economic burden of six common cancers in Gansu Province,and to evaluate the influence on individual,family and society.This will provide basic data and scientific basis for planning preventive measures and assessing the effect of cancer control.Methods Firstly,a cluster sampling method was conducted to collect medical record data of patients with colorectal cancer,breast cancer,liver cancer,esophageal cancer,stomach cancer and lung cancer from 2009 to 2011 in eight different hospitals in Gansu province.Then,direct medical cost would be analyzed.Secondly,face to face interviews to common cancers' patients and their families in seven different hospitals in Gansu province was used to evaluate the self-report direct,indirect medical cost and indirect economic burden.Results 1.Based on the medical records,the direct medical costs in Gansu province for colorectal cancer(4685.90 Yuan)were the highest,followed by breast cancer(4351.42 Yuan),liver cancer(3653.65 Yuan),esophageal cancer(3488.90 Yuan),stomach cancer(3109.46 Yuan)and lung cancer(2642.53 Yuan).Based on the face to face interview,the highest direct medical costs were for individuals with esophageal cancer(15843.44 Yuan),followed by stomach cancer(15046.68 Yuan),colorectal cancer(14626.74 Yuan),liver cancer(14388.69 Yuan),lung cancer(11764.31 Yuan)and breast cancer(11375.29 Yuan).The direct medical costs for individuals were far outweigh of the direct medical costs,and the percentage growth was 74.16%.2.Based on the face to face interview,the direct non-medical costs for individuals in Gansu province of esophageal cancer(1904.11Yuan)were the highest,followed by liver cancer(1279.56 Yuan),breast cancer(1179.38 Yuan),colorectal cancer(1151.05 Yuan),stomach cancer(1126.85 Yuan)and lung cancer(1119.50 Yuan).3.The indirect economic burden for individuals in Gansu province with lung cancer(4079.34 Yuan)were the highest,followed by breast cancer(3214.08 Yuan),esophageal cancer(2928.13 Yuan),colorectal cancer(2522.43 Yuan),liver cancer(2477.54 Yuan)and stomach cancer(2365.40 Yuan).4.The total household economic burden for individuals in Gansu province of esophageal cancer(20675.68 Yuan)were the highest,followed by stomach cancer(18538.93 Yuan),colorectal cancer(18300.22 Yuan),liver cancer(18145.79 Yuan),lung cancer(16963.15 Yuan)and breast cancer(15768.75 Yuan).5.Based on the medical records,the social direct medical burden for these six common cancers was 108.94 million Yuan per year.For the social economic burdens,lung cancer(25.13 million Yuan)was the highest,followed by stomach cancer(22.55 million Yuan),colorectal cancer(21.07 million Yuan),breast cancer(15.28 million Yuan),liver cancer(14.35 million Yuan)and esophageal cancer(7.17 million Yuan).In addition,based on the face to face interviews,the self-report social direct medical burden of the six common cancers was 421.51 million Yuan.And the social economic burdens per year was also highest for lung cancer(1.12 trillion Yuan),which was followed by stomach cancer(109.12 million Yuan),colorectal cancer(65.78 million Yuan),liver cancer(56.49 million Yuan),breast cancer(39.94 million Yuan)and esophageal cancer(32.57 million Yuan).6.The total family economic burden caused by the six common cancers was 545.96 million Yuan per year.For different cancers,the family economic burden in descending order were cancers of lung cancer(1.61 trillion Yuan),stomach cancer(1.34 trillion Yuan),colorectal cancer(82.30 million Yuan),liver cancer(71.24 million Yuan),breast cancer(55.36 million Yuan)and esophageal cancer(42.51 million Yuan).Conclusion These six common cancers create a huge economic burden to patients,their family and the society.Therefore,it is significantly needed to carry out early detection and treatment for common cancers to improve their quality of life.
