Objective The indexes of cancer mortality, potential years of life lost(PYLL), direct economic burden and indirect economic burden are used to analyze the demage degree of female common cancers. This will provide basic data and scientific basis for planning preventive measures and reducing economic burden of common cancers among women.Methods Taking advantage of the second and third all death survey in Gansu province, standardmortality rate of cancer, potential years of life lost(PYLL), average potential years of life lost(APYLL), working years of potential life lost(WYPLL), average working years of potential life lost(AWYPLL) were used to explore the disease burden and indirect economic burden of female cancers. And the cluster sampling method was conducted to collect medical record data of patients with cervical cancer, ovarian cancer, breast cancer and endometrial carcinoma from 1996 to 2006 in a certain general hospital and a special hospital in Gansu province. The average growth rate was used to analyze the trend of hospitalization expenses and to predict.Results (1) The average hospitalization expenses of cervical cancer, ovarian cancer, breast cancer and endometrial carcinoma were¥6025.60,¥4677.35,¥9332.54,¥6760.83, respectively. There is statistical significance among the four cancers (F=166.586, p=0.000<0.05) with one-way ANOVA, which suggested that the expense of breast cancer was the highest, while that of ovarian cancer was the lowest. (2) The daily average cost of cervical cancer, ovarian cancer, breast cancer and endometrial carcinoma were¥257.04,¥416.87,¥380.19, Y407.38, respectively. And the difference had statistical significance (F=102.791, p=0.000<0.05) with one-way ANOVA, which meant that the daily cost of cervical cancer was the lowest. (3) Medicine cost accounted for a dominant proporation of the whole cost for four common cancer and were 35.12%,53.93%,56.91%,45.14%, respectively. During the period of 1996 to 2006, the average growth rate of operation fee was increasing fastly among the four common cancers, which were 27.05%,39.11%,25.35%,39.19%, respectively. (4) The average hospitalization costs of cervical cancer, ovarian cancer and endometrial carcinoma exhibited a visible ascend trend expect for breast cancer. With the average growth rate of 8.06%,7.45%,6.47%, the expenses of the three types cancer will arrive at¥11163.74,¥10582.53,¥10818.86, respectively in 2010. (5) The daily costs of four common cancers also exhibited an increasing tendency. To 2010 the daily costs will arrive at¥604.17,¥1000.51,¥533.97,¥607.51 with the average growth rate of 13.15%,11.47%,4.67%,8.10%. (6) During the period of 1990 to 1992 and 2004 to 2005, the AWYPLL of cervical cancer were 7.16 person-years and 10.62 person-years and the indirect economic burden were¥19678.10 and¥84426.53, respectively. At the same time, the AWYPLL of breast cancer were 8.68 person-years and 12.94person-years and the indirect economic burden were¥20973.75and¥84795.20, respectively. (7) In Gansu province the adjusted mortality rate of female tumors were 105.13/100,000 and 105.33/100,000 from 1990 to 1992 and 2004 to 2005. Among all female cancers the adjusted death rate of cervical cancer was 11.88/100,000 and 5.70/100,000, which exhibited reducing tendency. And the adjusted death rate of breast cancer was 2.53/100,000 and 3.71/100,000, which exhibited an ascend tendency. (8) From 1990 to 1992 the PYLL of cervical cancer and breast cancer were 7985.50 person-years and 1973.50 person-years, while the APYLL of the two cancers were 18.40 person-years and 20.14 person-years. From 2004 to 2005 the PYLL of cervical cancer and breast cancer were 1291.88 person-years and 1047.88 person-years, while the APYLL of the two cancers were 14.68 person-years and 16.63 person-years. Both PYLL and APYLL were showing descending trend.Conclusion Both disease burden and economic burden of female common cancers are relatively high. Breast cancer and cervical cancer are the key problem for women, which will become the working emphasis in the future cancer prevention. Therefore, in order to improve female quality of life, it is significant and necessary to adopt some proper prevention measure positively.
