Objective:Analysis training intervention effect based on the theory of Kirkpatrick’s Four levels model of Clinical Nurse Specialists, and put forward feasible suggestions in order to complete training effect evaluation mechanism to provide the reference and basis.Methods:It is a test-retest and quasi-experimental research design. Sixty-two clinical nursing staff working in third class A hospital were recruited, who participated in the training of critical care specialist during 2014-2015.All of them received six-month critical relevant theoretical knowledge and clinical practice skills training. Among critical theory knowledge training for five months, a total of 62 hours and one months of critical care unit(ICU) in clinical practice. The research process divided into three parts. The first part: the baseline survey, before the intervention, using the general information questionnaire collection the general situation of information of participation nurse. meanwhile,investigation behavior attitude and the theory of the examination of the clinical practice ability of the 62 participation nurses. The second part: the implementation of the training.For 62 clinical nurses carry out six-month training intervention of critical relevant theoretical knowledge and skill. The training content included the clinical common critical content of relevant theoretical knowledge and behavior attitude. the training methods included theory teaching and clinical practice. The third part: the training effect evaluation.Based on the theory of Kirkpatrick’s Four levels model, from the reaction layer, learning layer, behavior layer, results layer to evaluate training effect of clinical nurses. The reaction layer to analyze training satisfaction with satisfaction survey questionnaire;Learning layer is mainly through analysis the theory and skills examination results ofbefore and after the intervention, the influencing factors to evaluation intervention effect;Behavior layer adopts the questionnaire analysis intervention effect of before and after of behavior attitude and clinical practice ability. Results layer by department head nurse to understand the performance appraisal and analysis clinical nursing service influence of the participation nurse in before and after the training. The questionnaire of this study was based on consulting a large number of literature combined with the actual situation of the training course design, and test reliability and validity. Questionnaire including general information questionnaire, training satisfaction survey questionnaire, critically ill health nurse behavior attitude and ability of self-evaluation questionnaire and his evaluation questionnaire, clinical nursing service quality assessment scale SPSS18.0 was adopted for data entry and statistical analysis. Statistical methods included descriptive statistics,Nonparametric rank and test, t test, single factor analysis of variance, Spearman correlation analysis.Results:Before training, the average score of the test about critical knowledge among sixty-two the participation nurses were 79.65±4.59, the average score of clinical skill was82.47±2.51.Before training, the average score of the behavior attitude was 92.08±8.59, the average score of the position ability was83.1±9.61, the score of each dimension of behavior attitude from low to high followed by the innovative spirit, professional knowledge, behavior normative, professional dedication. the score of each dimension of post ability from low to high followed by scientific thinking ability, education mission ability, problem solving ability, emergency ability and the team cooperation ability.Single factor analysis of variance showed that the average score of behavior attitude and clinical work ability among clinical nurse had significant differences(P<0.05) among different age, education, work department, work experience.Reaction level effect assessment of critical care specialist nurses to evaluate the participation nurse on training teachers, training content and training organization gave high evaluation, satisfaction is higher.Multiple linear regression analysis: the general information(a total of 11 factors)of the participation nurse as the independent variables, behavior attitude and post ability of critically ill health nurses as a dependent variables, for multiple linear regression analysis obtained: the work expenence and the first record of formal schooling is critically ill health nurses are the main factors causing the behavior attitude and the ability to post, the difference was statistically significant(P<0.05).Learning level effect assessment of critical care specialist nurse: before training, the average score of the test about critical knowledge among critical care specialist nurse was79.65±4.59,after training, the average score increased to94.98±4.14. before training, the average score of the test about clinical practice ability was 82.47±2.51,after training, the average score increased to 95.26±2.12,the difference was statistically significant(P=0.000).Spearman correlation analysis of the exam result after the training and general information found the age and education of the participation of nurses between exam result have correlation(P<0.05).Behavior level effect assessment of critical care specialist nurse: before training, the average score about behavior attitude among critical care specialist nurse was 92.08±8.59,after training, the average score increased to 95.45±7.29;before training, the average score of the test about post ability among critical care specialist nurse was 83.1±9.61, after training, the average score increased to 90.18±9.99, the difference was statistically significant(P=0.000).Result level effect assessment of critical care specialist nurse: before training, the average score about clinical nursing service quality among critical care specialist nurse was95.34±5.34,after training, the score increased to 97.04±3.89,the difference was statistically significant(P<0.001).Conclusion:To develop the program of critical care specialist nurse training can improve the critical relevant theoretical knowledge and clinical skills about clinical nurse.Based on the Kirkpatrick’s Four levels model to evaluation the training effect of the critical care specialist nurse program can all-around comprehensive evaluation of training effect. Meanwhile encourage the participation nurse can apply knowledge and skills to practice clinical work, improve the quality of nursing service.
