| Since 1970s, in order to achieve the global strategic objectives of "Health For All", China has made continuous effort to exploration and innovation of medical and health system, trying to solve the health care problems.Rural health services integration,or the integration of township and village, as an important part of the medical and health system reform in the nation, is not only one of the measures to ensure primary health care and primary health institutions reform in rural areas,but also plays a crucial role in realizing the goals of "Health For All" in China.From 1990s, it has made great progress in improving the allocation of health care resources and alleviating the poor competition between health care institutions in rural areas. There are,however,still many problems in practice, which are characteristic of unique across the country.Therefore,Take M city as an example,a representative of the minority area in China’s southwest. By researching on dilemmas of the rural health services integration in M city,the aims are to provide a theoretical basis for M city and even a realistic basis for the whole country.On the basis of the domestic and foreign literature, first of all,this paper illustrates the situation of the rural health services integration in M city with government documents, questionnaires and interviews;Secondly, with the Pareto chart, many problems belong to three levels,including the government, township hospitals and village clinics, and each level can be divided into three aspects of the main problems, minor issues and general problems,which are also analyzed.Finally, according to the three levels of problems,there are some countermeasures being taken to promote the rural health services integration in M city:1. Reform the top-level system,settling the village doctors’ troubles back at home; 2.Activating the internal and external mechanism,establish a unified financial model;3.Strengthen the corporation among the doctors between township hospitals and village clinics,promoting positive interaction;4.Improve the employment dismissal and assessment mechanism,stimulating the enthusiasm of medical staff.Through these measures,the aim is to optimize the allocation of health resources and achieve the goal of equalization of health services. |
