| Background: With the prevalence of chronic diseases,approaches of effectively controlling chronic diseases are gradually being paid attention,but interorganizational disease management services cannot be delivered by single health worker or single medical institution because of its complexity,so how to provide interorganizational full-cycle disease management services for patients with chronic diseases and how to promote interorganizational collaboration among health workers are the key to integrate health care in chronic diseases.Objectives: The purpose of this study is to study the mechanism of collaboration between county,township and village health workers,as well as to examine the effectiveness of interorganizational collaborative intervention programme through randomized controlled intervention experiments,and to analyse the facilitation and barriers of interorganizational collaboration among health workers in counties,townships and villages,and to put forward some suggestions on how to promote the interorganizational collaboration among health workers providing rural chronic diseases services.Methods: Firstly,this study adopts the literature inductive method to guide and construct the theoretical frame model of the study.Then based on the theoretical framework,the intervention scheme of sample area is formulated and the intervention research is carried out.After a year of interorganizational collaborative randomized controlled intervention experiment on chronic diseases in rural areas carried out in theintervention group,quantitative data are used to evaluate the intervention effect.Quantitative questionnaires are used to investigate health workers.A multi-stage random sampling method is used to sample the sample for questionnaire.The statistical methods is used including descriptive analysis and rank order testing.Results: The health information platform connected with county,township and village has not been established,which affects the improvement of interorganizational information transmission;interorganizational communication and cooperation has some improved effect,and regular team training has improved the familiarity of health workers;interorganizational collaborative performance awards are inadequate and lack of integration of external interests.Suggestions: Strengthening the government-led in building health information-sharing platform linking counties,villages and villages;exploring multiple forms of technical assistance and capacity-building to promote interorganizational exchanges and cooperation and to enhance the level of collaboration;increasing the level of agreement on interorganizational cooperation,and improving incentives for benefits. |
