Study On The Evaluation System Of The Clinical Competence For Master Of Nursing Specialist

It is necessary to training senior nursing personnel, as the development of the socio-economy and people’s needs for high quality of health care. In January 2010, the Academic Degrees Committee of the State Council sanction the setting of Master of Nursing Specialist(MNS) to cultivate nursing elites with high-level, application-oriented and professional ability for the clinical. The nurture and evaluation of clinical competence are the major contents for the education of MNS. Owing that the MNS education is emerging in China, evaluating a system for apprasing the clinical competence of MNS, as well as building the standards for MNS training are urgent and momentous problems.Objectives:Constructing an comprehensive, reasonable and easy-operation evaluation index system of the clinical competence, which can be utilized to make imputations for assessing MNS’clinical competence objectively and scientifically. Furnishing guidelines for MNS cultivation of clinical application.Methods:Questionnaires, Delphi and superiority chart were undertook to establish the evaluation system of the clinical competence for MNS. The cardinal contents and methods are as follows:1.The primary construction of evaluation system of clinical competence for MNS.The construction of the pool of questionnaire of clinical competency evaluation for MNS were based on the "nursing professional master degree graduate guidance training programme" instituted by Chinese Professional Degree Graduate Education Steering Committee.Literature analysis, interviews and questionnaires are instrumental in identifying the connotation of nursing clinical competence. The nursing specialists and medical education experts were selected in western China to set up the contents and structure of the evaluation system of the clinical competence for MNS by Delphi.2.The calculation of evaluation indicator weights of clinical competence for MNS.The evaluation indicator weights of clinical competence for MNS were calculated by optimizing timing diagram method, based on the results of Delphi.3.The tests of the reliability, validity and effects of application of the evaluation system of the clinical competence for MNS.42 MNS were selected from three medical colleges who have practiced in the clinical more than 3 months by convenience sampling method. Self-assessment scale based on the constructed system were answered by the 42 MNS to quantitate the reliability and validity of the evaluation system. Then the system were gave substance to the rotation of the Grade 2013 and Grade 2014 graduates in Chongqing Medical University. A satisfaction questionnaire of the evaluation system was created to exam 17 clinical teachers, making for understanding the usage of evaluation system and meliorating the clinical evaluation system.Results:1.The consist of the evaluation system of the clinical competence for MNS.22 nursing specialists and medical education experts from 2 nursing schools and five affiliated hospitals, completed two rounds of the consultation. The experts enquiry for the familiar with degree coefficient is 0.846,and the judgment coefficient is 0.934, authority coefficient is 0.892. Effective recovery ratings in two rounds are 100% and 81.8%,the average out rate of indicators are 55.4%and 58.0%, Kendall’s W coefficients were 0.195 and 0.182.The evaluation system including 5 first indexes (involving the theoretical knowledge, competence of clinical nursing, professional ethics together with the ability of research, education and management),13 second indexes and 48 third indexes was established.2.The weight of indicators in the evaluation system of the clinical competence for MNS.The study results ascertained the weight of the first indexes of evaluation system of clinical competence for MNS from large to small are the theoretical knowledge(0.264), competence of clinical nursing(0.239) professional ethics(0.231), research ability(0.156), education and management ability(0.111).3.The reliability and validity of the evaluation system of the clinical competence for MNS with its effects of application.Self-assessment scale based on the constructed system were completed by 42 MNS who have clinical practice more than three months.42 questionnaire were issued with a 85.7% effective rate. The statistical results manifested that Cronbach coefficient of the system was 0.889, five of firs-class indexes Cronbach coefficient were 0.882,0.885,0.854,0.873 and 0.894 respectively, split-half coefficient was 0.746,retest correlation coefficient was 0.805.17 teachers who have took this evaluation system participated in this satisfaction survey with a 100% effective rate. All 17 teachers regard this evaluation system as comprehensive and reasonable.16 teachers considered it is easy to operate with an accurate indicators description,15 teachers believe it can reflect MNS’clinical competence exactly.Conclusion:The assessment index system of clinical competence for MNS (including 5 first indexes,13 second indexes and 48 third indexes) formulated by Delphi is reliable and show a good reflection in the clinical application, it can be reflect the MNS’ clinical competence accurately and scientifically. This system corresponds to the requirements of training objectives for MNS, and provides a guidance for training and quality controlling of MNS.
