Objective By clinical research of the manipulation on neck-shoulder and head-face in the treatment of cervical spondylosis with the vertebral artery type, the thesis aims to prove that the manipulation has better effects, esp. in improving the symptoms of dizziness and vertigo, and to inquire into diagnostic significance of TCD and vertebral artery ultrasound to cervical spondylosis with the vertebral artery type, and to analyze effect and mechanism of the manipulation on neck-shoulder and head-face n in the treatment of cervical spondylosis with the vertebral artery type.Methods Select 70 patients of the cervical spondylosis with the vertebral artery type in accordance with the standard of this project. All the patients were diagnosed and treated from May 2013 to December 2014, in Song’s bone-setting department of affiliated hospital of Gansu University of TCM, Doll’s clinical random cases form is divided into two groups among whom,26 male cases, 44 female cases, aged from 23 to 70(average 47.2), with a disease course of 1 month to 2 years(average 6.37).For the treatment group,35 cases, 13 male cases and 22 female cases aged from 24 to 66(average 46.91), and for the control group, 35 cases, 13 male cases and 22 female cases aged from 26 to 61(average 47.14).Evaluate the clinical effectiveness of the 2 groups before and after the treatment, by indexes of clinical sign, vertigo symptom scoring, TCD and vertebral artery ultrasound.Apply SPSS17.0 to carry out statistics analysis.Results After treatment, 9 patients were cured, 20 patients showed excel ent in efficiency, 3 demonstrated effective and 3 with no response in the treatment group,effective rate of treatment was 91.43%. Five patients were cured, 19 patients showed excel ent in efficiency,4 demonstrated effective and 7 with no response in the control group,effective rate of treatment was 80.00%.Compare the total effective rate of the 2 groups, signif icant differences in two groups were observed(P<0.05).The grade comparison of clinical signs between two groups: It is no cleardifferences in joint movement degree, spinous process or parapophysis, while the treatment group is obviously superior to the matched group in soft tissue tenderness, occipital neuralgia, vertebral artery stress. The relief comparison of symptoms of dizziness between two groups: after treatment, symptoms have been relieved evidently. The manipulation onneck-shoulder and head-face group is obviously superior to the matched group in dizziness, mentality and social adaptability. It can be testified that, after treatment, patients in two different groups whose vertebral artery flow has been improved by the test from TCD and vertebral artery color Doppler ultrasound.Conclusion: a. Applying the manipulation on neck-shoulder and head-face to treat the cervical spondylosis with the vertebral artery type has a determined curative effect and is more practicable, which can be used as one of the preferred treatment methods, especially for the patients of apparent dizziness and insomnia. b. As manipulation on neck-shoulder and head-face values the concept of the whole, and based on the whole spine stability, treats both the muscles and the bones as well as meridians and acupoints, it is worth popularizing with a unique idea. c. It values healthcare education, making the patients, esp. the young pay more attention to the disease and realize the severity of it, so that stress should be put on the prevention of disease.Cooperate with function exercise to prevent recurrences. |
