| BackgroundThe nursing management system is an internal tool, which is normally used by hospitals to bound and coordinate their nursing staff’s behavior standards, procedures and methods.With the reform of China’s medical and health system, the humane care for medical personnel training has became an important element of the reform.However,due to the influence of traditional management mode, the "humanization" in nursing management system of China has not gone far enough,particularly the humanistic care to patients.Humanism as a soft skill of nurses, its implementation is completely dependent on the nurses’personal internalization and cultivation ,that is not enough. It should be settled from the institutional level, that is, establishing an perfecting scientific management system, using the system to discipline nurses ca-ring behavior, avoid the humanism caring in nursing behavior as "conscience".In view of this, this research will analysis the nursing management system of hospitals in our country, and to see whether it can meet patient demand for humane care.ObjectiveThis study had analyzed the nursing management system in our country, through expert interviews and text analysis methods,to concluded the institutionalization of humanistic nursing status and questionnaire results of Humanistic care for nursing needs of patients,explore effective coping strategies,improve care management system,raise the level of humane care system,standardized nursing care nursing behaviors and improve patient satisfaction.Methods1. Interview method Used the semi-structured expert in-depth interviews to find out the nursing management system issues and the institutionalization of humanistic nursing management.According to the results of the literature review and expert interview and expert’s advices on the development of humanistic nursing care system benchmarks.2. Data collection Collected the content nursing management system of tertiary hospitals’ and secondary hospitals’,which had achieved national certification, through the Baidu and Google and other famous search engine;Collected the content nursing management system of hospitals through field investigations.3. Text analysis method Based on the humane care standards which were summarized by Professor Liu Yilan, to develop standards of care in this study are as follows:Communicate with patients; Show interest, respect, courtesy and attention to patients and their families; Let patients and their families feel comfortable and safe; Psychological concerns of patients, encourage and support patients. Analysed the nursing management system,to find out the current nursing humanistic care institutionalization. In accordance with the standard of nursing care behavior criterion of humanistic care system.4. Patients needs of humanistic careSubjects 400 patients who met the inclusion criteria in hospitals which have been field investigated.Questionnaire The questionnaire includes two parts:General information:patient’s gender,age,residence, income, educational level,marital status;Patients needs of humanistic care;through literature compilation review ,reference Caring Behavior Assement scale for the initial questionnaire, modify by experts, eventually form the questionnaire which including 20 items questionnaire of hospitalized patients’needs toward caring. By small sample surveys, this questionnaires’ Cronbach’s a was 0.919.Questionnaires The humanistic care for nursing needs of patients with a questionnaire to survey patients met the inclusion criteria, questionnaire issue don the spot, recovered at the scene.Results1. Interview result According to the results of 15 interviews with experts, refined the theme:current nursing management system in hospital;Ten core system of nursing reflected humanistic care;The nurse’s care behaviors was less;Hospitals had pied Attention to Humanistic care.2. Material collected General and text analysis results This survey collected 65 hospitals’ nursing management system.Among them,there were 37 tertiary hospitals,and 28 secondary hospitals.The current nursing management system of hospitals were somehow reflecting the caring for patients.There were 54.1%,45.9%,45.9% tertiary hospitals and 64.4%,42.9%,42.6% secondary hospitals reflects the humane care of patients in the ward management system, grading care system health education system.While in other seven core systems, there were only 3.6%-29.7% hospitals reflected the humane care .In two care content,which were "the Nurses’ under doctor’s orders and checklists, inform the patient and inform precautions, seriously ill patients and those with reduced mobility escort someone should check to ensure patient safety "and "emphasize the attitude of nurses to check security risk level of care ", the number of tertiary hospitals and secondary hospitals, P<0.05, the difference was statistically significant.3. Patients needs of humanistic care There were 400 questionnaires.394 valid questionnaires were collected. The effective recovery rate was 98.5%.Inpatients demands for 20 humanistic care scores were higher than average, that patients were more demand for humanism care. The level of hospital, gender and patients’ calligraphic location made a significant difference towards caring needs score, P<0.05.Age, marriage status, level of education and income didn’t make significant differences toward caring needs score, P>0.05.4. Nursing humanities institutional analysis The survey found that self-esteem and love and belonging were more demanded by patients, but the core of the current management system of hospital care in this regard reflects the humane care less, can not meet the needs of patients.ConclusionOur 10 core nursing management system in hospitals had reflected the humanity solicitude, and the institutionalization of humanistic care status in tertiary hospitals’ were better than secondary hospitals’, current status of institutionalization of humanistic nursing care cannot meet the patients needs, care management system still need to be improved. |
