The Survey Of Knowledge, Attitude, Behavior To Medical Staff In Prevention Of Hepatitis B In Highest Level Hospital Of Six Cities In China

The doctor is a special occupation, the medical accident is most happen to themselves in the practice. According to a report stated that the Hepatitis B Virus (HBV) infection rate of medical staffs is higher than the public, and HBV is the main microorganisms which threaten to the medical staffs. The infection rate of HBV is higher in our country, the medical staffs and the patients can be infected each other in the practice. It is important to prevent HBV for the medical staffs.Therefore, it is necessary to conduct a survey of knowledge, Attitude, Practice (KAP) of preventing HBV in the medical staffs. We can know the KAP status of the medical staffs through the survey, explore the risk factors that caused by the practice, find the measures of preventing HBV in the medical staffs, and to protect the medical staffs and patients.[Object] To know the KAP status of the medical staffs in six cities; to know the main factor which can effect KAP; to provide basis on training and health education in preventing HBV.[Methods] The cities of Beijing Shanghai, Guangdong, Sichuan, Yunnan and Gansu province were been selected, and1-2hospital of high level were been selected in each city. Sampling methods:in the selected hospital, the departments selected as followed: Department of Surgery, Department of Operation, the Hemodialysis Room, Laboratory, Department of Stomatology, Department of Infection, Department of Liver Disease, Department of Emergency, Department of Pediatric, Department of Obstetric, Department of Administrative management, and etc.. The survey objects included the medical staffs of the Department selected. Analyze the information using SPSS16.0.[Results] The medical staffs of a higher academic qualifications, higher professional title, longer time working, occupation is a doctor, at the age of30~and40~is higher scores in the KAP knowledge of prevention and treatment in Hepatitis B. The results showed that the Epidemiology knowledge is poor,79.60%of the medical sataffs think that Hepatitis B carriers is the infection Source of HBV,77.53%of the medical sataffs think that Perinatal transmission is the transmission route of HBV, which is lower than the target involved in the program of2006-2010national Hepatitis B prevention and control, in which the awareness of Hepatitis B prevention and control is higher than80%. Some medical sataffs do not know the reported criteria of Hepatitis B carriers and Chronic Hepatitis B.36.65%of the medical sataffs think that Hepatitis B carriers should be reported in China Information Symtem for Disease Control and Prevention, and66.46%of the medical sataffs think that Chronic Hepatitis B should be submitted it. On the hepatitis B diagnostic criteria,6.05%of the medical staffs knew the diagnostic criteria of acute Hepatitis B;2.84%were able to correctly grasp the diagnostic criteria of chronic hepatitis B;21.90%can be correctly diagnosed hepatitis B carriers. The medical staffs of different characteristics are generally poor in the diagnostic criteria of Hepatitis B, the awareness rate of the staff in high degree, high professional title and the doctor are higher than other groups, and the awareness rate of the doctor is higher in diagnostic criteria of acute Hepatitis B.The Coverage rate of Hepatitis B vaccine of some high risk department is lower than the others, the results as followed:Department of Surgery(90.6%), Department of Operation(84.2%), Department of Stomatology(93.80%), Department of Infection(91.30%), Department of Liver Disease(93.90%), Department of Pediatric(92.90%), Department of Obstetric(92.20%), Department of Emergency(82.00%), the Hemodialysis Room(72.30%), Laboratory(71.20%), Department of Health Care (70.60%), Department of Administrative management(74.00%). For the result, the main reason is no one to organize the activity and the medical do not know to vaccination instead of the vaccine price.The factors that effect immunization of HepB are the area, the title, the history of stab wound, and the people who know the susceptible population of Hepatitis B, which are the focus on training in the future.[Conclusions] The epidemiology knowledge of Hepatitis B for the medical staff need to be improved, and to enhance the training to the targeted medical staff, especially to the diagnosis criterion and reporting of Hepatitis B; to improve the Hepatitis B vaccine immunization rate, and to prevent the Hepatitis B virus from medical staff.
