BACKGROUD"Cancer Registration in China2012Annual Report" display:National annual new cancer cases is about3.12million cases; has an average of8550people a day, every minute there are six people are diagnosed with cancer,every7-8will have one people died of cancer.The nursing career has achieved remarkable development in the "Eleventh Five-Year" period in China.The number of nursing teams has a significant increase while the provinces (autonomous regions and municipalities) develop standardized training for specialist nurses according with the requirements of the Ministry of Health.The necessity of personnel training for specialist nurses has reached consensus understanding at present.It is important to focus on specialization training for the nurse practitioner in the Specialist nursing field.With the continuous development of the domestic oncology care,"Chinese Nursing Career Developent Plan2011-2015" clear to establish five national oncology nursing professional training base to train more clinical nurse specialists before2015.The accreditation standards is the foundation and key for establishing a qualified clinical training base to ensure the quality of training.Specialist nurses (clinical nurse specialist, CNS) is formed and developed in the process of professionalization to be a senior clinical care workers, who can provide core care to support the cancer patients in a multi-disciplinary team At present, the cultivation for specialist nurses mainly for hospitals cultured alone or co-culture of hospitals and schools. And the hospitals are both as a practical skills training base. Actually,the training for cancer specialist nurses is still in its infancy, no uniform standards.The common feature of these is the training base are Grade III-A Hospitals or specialist hospital, but the specific hardware and software standards is different. Its admission standards are mostly developed by themselves, the training mode and training conditions is uneven.It is difficult to ensure training quality.The exact figures of CNS are difficult to statistics in domestic, there are1000-2000people who may have received standardized training. Although the country has carried out about the cancer specialist nurse short-term culture. However, due to the lack of a unified base of assessment criteria, it has been a bottleneck to the healthy development of oncology. Meanwhile, It has no the report about the accreditation standards for oncology nursing professional training base at present.This study intends to develop a accreditation standards for oncology nursing professional training base to ensure the quality of training oncology nurses as the grim situation of serious threat to the health and the status quo of oncology nursing specialist.OBJECTIVEThis study aims to build a scientific and feasible accreditation standards and its weights is appropriate to the oncology nursing professional training base in dometic by using Delphi method and Analytic Hierarchy Process (AHP).And measured by the analysis of a small sample to verify the quality. This study view to provide scientific, standardized, objective, impartial measurement tools for the accreditation to the training base, so that it can facilitate the training to be systematic and standardized for the specialist nurses.STUDY SUBJECTS AND METHODSThe expert consultation questionaire for accreditation standards for oncology nursing professional training base is formed on the basis of the literature review and expert interviews in the early. And it is also discussed about the basic principles of construction for the system and refer to the qualification requirements made for APN (Advanced Nursing Practice Nurse)by Hong Kong Hospital Authority and the Guangdong Provincial Cancer specialist nurses in clinical practice training base assessment criteria, combined with the status quo of oncology nursing specialist before formation. This study collates, statistics, analyzes the results and options and combined with expert feedback to establish the accreditation standards initially for oncology nursing professional training base after three expert advice.Fisrtly, compared the means of indicators in the Delphi to determine saaty scale,and constructed judgment matrix; Secondly, the square root method is used to determine the weight and the maximum eigenvalue. Sorted single level and whole system and its consistency inspection; Finally, carried out in chronological order according to their size and determined the weights in all levels.This part has tested the reliability and validity of the accreditation standards for oncology nursing professional training base, and invited5training base which has been certified to use the " accreditation standards (draft)",for statistical analysis to verify its quality.The data was used analyzed by using SPSS13.0software and ExceI2007vertion. Descriptive statistics was used to analyze demographic constitution of experts. Using expert positive coefficient, authoritative coefficient, concentration of expert opinion, the level of coordination and other indicators to examine the reliability and representation of expert consultation. The weight in all levels was determined by using Analytic Hierarchy Process (AHP) and expert scoring method. Using KR-20, face validity, content validity to verify the quality" accreditation standards (draft)".RESULTS1. The results of the Delphi method for construction of accreditation standards for oncology nursing professional training baseThe three rounds has consulted with21experts. They are the Medical nursing educators, nursing management experts in Grade Ⅲ-A Hospitals, clinical oncologist, oncology clinical nurse specialist.They come from Guangdong, Jiangsu, Sichuan, Fujian, Jilin five provinces10cities, including two medical university and17Grade Ⅲ-A Hospitals.The average age of experts in42.95±6.09years, work experience on average15.81±5.16years, They both have a college degree or above, the intermediate title. Dr.5(23.8%), master3(14.3%), undergraduate12(57.1%), college1(4.8%), are senior eight (33.3%), the Deputy High11(57.2%).Intermediate2(9.5%).The questionnaires were distributed21copies in three rounds of survey. And the valid questionnaires were21,18,20, the effective rate was100%,86%,95%. There are75recommendations presented by14experts in the three rounds of survey.76%of experts can complete consultation questionnaire within the time. Experts enthusiasm is higher overall. The authority coefficient is0.7-0.95, with an average of0.825to the accreditation standards. It can be seen that a high degree of authority of this study, and the results are credible and reliable. The coordination coefficiente were0.181,0.170,0.196in three rounds of survey, statistically significant (P<0.05), explained that the expert opinions are consistent and has a higher degree of consistency in the accreditation standards.The screening criteria of the entries are the importance assigned mean were>3.5, coefficient of variation<0.25, selectivity>70%. The accreditation standards for oncology nursing professional training base which has100index including the basic conditions from the hospital, department of basic conditions of clinical practice teaching teachers, organizing management, oncology nurse these five areas was determined by combining with expert advice and research group discussions. It includes5first-class indexs,19second-class indexs,76third-class indexs (17A-level indexs,20B-level indexs,39C-leveli indexs)2. Using AHP and expert scoring method to determine the weight of accreditation standards for oncology nursing professional training baseThe results show that the first-class indexs including the basic conditions for hospitals, the basic conditions for department, the faculty in clinical practic eteaching, training management, oncology nursing.The weight of them were0.0849,0.1213,0.2484,0.1600,0.3855.The four items " specialist operating techniq ues","specialist nurses","training rules and regulations","training programs"has large proportion in the weight of the the second-class indexs were:0.0680,0.0 680,0.0776,0.0824. According to the judgment of the importance for each entry in the third-class indexs by experts, we divided into A, B, C grade.The judg ment rule is to achieve "B-good" file, must be in line with "C-pass" requireme nt profile, go to "A-Excellent" must be met "B-good" file requirements.The res ult of total level of single-sort and sort in all levels of indicators has satisfact ory consistency by test.3. The quality test results of accreditation standards for oncology nursing professional training baseThe" accreditation standards" includes3level and100indexs, acovering all aspects of oncology nursing professional training base after consultion. Its reliability coefficient was0.985, CVI value of0.815, has a good face validity and content validity by the analyze of SPSS13.0statistical software afer consultation.This part has invited the training base which has been certified to use the " accreditation standards (draft)" and make comparative analysis. The results showed consistent with its previous assessment results, so that it indicate the science of this" accreditation standards" is strong. Meanwhile, the accreditation standards of this study not only applies to the provincial training base, also applies to municipal, national training base. It’s applicability is stronger.CONCLUSIONS1. Its weight showed that the core evaluation criteria investigated indicators is oncology nursing. A high quality in oncology nursing is the important guarantee for the quality of training bases.3. The accreditation standards for oncology nursing professional training base has a scientific evaluation is of great significance to the evaluation for the training baseThe Construction of accreditation standards for oncology nursing professional training base has a good quality after empirical research.Today the country has not been uniform and scientifically validated oncology nursing professional training base accreditation standards reported in the literature.This study can enhance the responsibility of oncology nurse through a clear definition and qualification requirements sense of responsibility. In addition, the regulate of oncology nursing assessment can ensure the quality of training for specialist nurses and provide a reasonable tool for the establishment of oncology nursing professional training base or other training bases and scientific evaluation of the training base.
