Objective:In view of the current situation of severe shortage of nurses in hospitals,combined with the new situation of stratified setting and management in international nursing industry,the training of auxiliary nursing personnel has gradually attracted the attention of national,government and industry scholars in recent years.Integrated use of various methods of this research,in order to primary nursing assistants training needs as the guidance,based on post competency force,to the principle of clinical need,building a set of suitable for primary nursing assistants training course content system,and analyzed by using analytic hierarchy process(AHP)to calculate the index weight.To provide theoretical support for the guidance of nursing staff training in our province,at the same time,it also provides reference for the training of auxiliary nursing staff in other areas.Research Methods and Contents:1.Questionnaire survey: Self-compiled questionnaire was designed to investigate the general information and training needs of nurses who had received systematic training and worked in the hospital for more than 6 months.2.Semi-structured interviews: By means of one-to-one and face-to-face semi-structured interviews,the clinical nursing,nursing management and nursing education experts involved in the training of nursing assistants were interviewed to understand the problems and Suggestions on the improvement of the training courses for nursing assistants in our province.3.Literature analysis: Based on the preliminary questionnaire survey,expert interview results and relevant literature summary,the content system of the training course for primary medical care assistants was preliminarily constructed.4.Delphi method: Two rounds of correspondence were started for the preliminary construction of the training course content system.At the end of each round of letter consultation,the index content was screened and revised,and the index system content was finally determined.5.Analytic Hierarchy Process: AHP was used to calculate the weight of each index and to test the consistency of the final course content index system.Results:1.Results of the questionnaire survey:(1)The nursing assistants team is still dominated by female farmers,with a low level of education.Almost none of them had ever been engaged in nursing related occupations before they participated in the training of nursing assistants,with an average age of 41±6 years;(2)Among the nurses surveyed,except for almost all of them who have participated in the training of life care skills,less than half of them have received professional training of clinical care content,and the least number of them have received the training of professional ethics,laws and regulations and communication skills;(3)The training needs of nursing assistants are still dominated by life care skills.Secondly,the training needs of ward arrangement,on-job guidance and professional care are also relatively high.A considerable number of people also express that they need to receive the training of working process and psychological care knowledge;(4)The training period should be 1-3 months based on the nursing staff's endurance and comprehension ability.2.Results of semi-structured interview : The problems existing in the content of the current training courses for nursing assistants in our province and the opinions on future training can be summarized as the following points:(1)A unified training textbook has not been formed,and the proportion of training course contents is unreasonable;(2)In the face of the less educated training population,attention should be paid to quality and safety education;(3)Due to different departments and different needs,the training time should be appropriately extended;(4)A qualified nursing assistant should be qualified in attitude,knowledge and skills.3.Preliminary constructed the index system of primary health nurse training course content: Using the Delphi expert enquiry method,the selection from shanxi,Beijing,guangzhou,Shanghai,Qingdao and other five areas of 28 experts involved in the enquiry.Finally,the training course content system with basic quality,basic knowledge and basic skills as the first-level indicators,as well as 6 second-level indicators,21 third-level indicators and 85 fourth-level indicators under the 3 first-level indicators was determined after two rounds of enquiry for experts.Two rounds of questionnaire effective recovery rate were 100% and 96% respectively,both over 70%.In the two rounds of consultation,a total of 21 experts put forward Suggestions for revision,and the authority coefficient of the overall expert group was 0.865.The results of the second round of consultation showed that Kendall's ? of level four items was 0.158,0.511,0.313 and 0.353,expert opinion is more consistent.4.Analytic Hierarchy Process(AHP)to calculate index weight at all levels and conduct consistency test: The weight of the first-level indicators is ranked from the largest to the smallest in order of basic skills(0.5396),basic qualities(0.2970)and basic knowledge(0.1634).Among the secondary indicators,life care skill has the largest weight value,followed by occupational quality,medical care skill,clinical care knowledge,physical and mental quality and basic medical knowledge.In the third index,discharge care,professional ethics,prevention and cognition of patients' safety hazards rank in the top three.When CI < 0.10 of each index,the logicality is good,and the result of setting index weight value is acceptable.CR values were also all < 0.10,indicating that the consistency of the judgment matrix was satisfactory,and the overall sequencing consistency was good,which could pass the consistency test.Conclusion:Through two rounds of Delphi expert letter consultation,the course objective was finally achieved from three aspects of attitude,knowledge and skills.The training course content system is constructed with 6 second-level indicators,21 third-level indicators and 85 fourth-level indicators under the 3 first-level indicators of basic quality,basic knowledge and basic skills.Experts are highly motivated,authoritative and well coordinated,and the research results are authentic and reliable.Weight setting and analysis were carried out for each index,and the result of weight setting was acceptable.It can provide theoretical basis and reference for the training courses of nursing assistants in the future.
