Construction Of The Training Index System For ICU Nurses In PADIS Management Of Patients

ObjectiveTo construct the training index system for ICU nurses in PADIS management of patients which aims to provide theoretical basis for ICU education and managers carrying out nursing staff training on pain,agitation,delirium,immobilization and sleep interruption(abbreviated as PADIS).In addition,improving the ability of ICU nurses to comprehensively manage pain,agitation,delirium,mobility,and sleep interruption in critically ill patients,as well as reduce the pain of critically ill patients,and increase the treatment rate.MethodsFirstly,a research team was established,and the first draft of the PADIS evaluation index system for ICU nurses was drafted through literature research methods,expert interview methods,and research group discussions.Then the Delphi method was used to determine the PADIS training index system for ICU nurses through two rounds of expert letters.The analytic hierarchy process is used to analyze and calculate the weights and rankings of indicators at all levels.Result(1)Expert enthusiasm:17 questionnaires were sent in the first round of letter inquiries,and 17 were retrieved.The effective recovery rate was 100%.4 experts put forward suggestions for revision of the letter.In the second round of letter inquiries,17 questionnaires were sent and 17 were retrieved.The effective recovery rate was 100%.The effective recovery rate for both rounds of letter inquiries was 100%,indicating that the experts were highly motivated.(2)Degree of expert authority:The first round of 1 experts’ judgment coefficient is 0.935,familiarity is 0.771,and the authority factor is 0.853.The second round of experts’ judgment coefficient is 0.924,familiarity is 0.724,and authority coefficient is 0.824.Both are greater than 70%,indicating a high degree of expert authority(3)ICU nurses’ training index system for PADIS management of patients:the system includes 3 first-level indicators,14 second-level indicators and 58 third-level indicators.The first-level indicators are PADIS knowledge,PADIS skills,and PADIS comprehensive application ability.The second-level indicators are analgesia knowledge,sedation knowledge,delirium knowledge,restraint knowledge,early activity knowledge,sleep interruption knowledge,analgesia assessment and monitoring,sedation assessment and monitoring,delirium prevention、monitoring and nursing,early activities,sleep interruption preventionn treatment and nursing,the ability to grasp the key points of nursing,critical nursing thinking,humanistic care and nursing.After the second round of correspondence,the average value of the importance of the first-level indicators is between 3.88-4,the coefficient of variation is between 0～0.086,and the full score rate is between 88.2%～100%;the importance of the second-level indicators is between 3.59～3.94,the coefficient of variation is between 0.062～0.215,and the full score rate is between 58.8%～94.1%;the mean value of the importance of the three-level indicators is between 3.53～3.88 points,and the coefficient of variation is between 0.086～0.221.Among them,the full score rate is between 41.2%～88.2%.(4)Weights of indicators at all levels:The weights of the three first-level indicators are:PADIS knowledge(0.493),PADIS skills(0.196),and PADIS comprehensive application ability(0.311).The weights of 14 secondary indicators are:analgesia knowledge(0.043),sedation knowledge(0.034),delirium knowledge(0.024),restraint knowledge(0.024),early activity knowledge(0.060),sleep interruption knowledge(0.060),and pain assessment and monitoring(0.013),sedation assessment and monitoring(0.009),delirium prevention,monitoring and care(0.007),early activity(0.005),sleep interruption prevention,treatment and care(0.004),ability to grasp nursing priorities(0.039)),critical nursing thinking(0.019),humanistic care and nursing(0.039).Conclusion(1)The ICU Nurses’ Training Index System for PADIS Management of Patients constructed in this study has scientific and rigorous research methods,true and complete index content,and reasonable and reliable research results,which can provide theory for ICU nursing managers and educators to carry out relevant training.(2)This training index system aims at the lack of PADIS knowledge that is common among ICU nurses,and regards PADIS knowledge training as the top priority of the training system,which can improve the theoretical framework of ICU nurses’ knowledge of PADIS management of patients,and further improve the level of management of patients with PADIS of ICU nurses.
