Effects Of Jianxin Granule On The Plasma Monocyte TLR4and Plasma TNF-α In Patients Who Belong To Deficiency Of Heart Qi、Blood And Water Stasis Syndrome Type Chronic Heart Failure

Objective:To observe curative efficacy of Jianxin Granule in patients who belong to deficiency of Heart Qi、Blood and Water Stasis syndrome type chronic heart failure. To investigate the effect of Jianxin Granule on the expression of plasma monocyte TLR4and plasma TNF-ain patients who belong to deficiency of Heart Qi、Blood and Water Stasis syndrome type chronic heart failure,and preliminary study the action mechanism on chronic heart failure with Jianxin Granule.Methods:Dividing the72patients who belong to deficiency of Heart Qi、Blood and Water Stasis syndrome type chronic heart failure into a control group and a treatment group randomly, each include36patients. The control group take Irbesartan75mg and Bisoprolol1.25mg and Hydrochlorothiazide12.5mg per day. Adjusting the doses per week to the target doses according to the patients’condition and add other diuretic like furosemide to control the patient’s condition in the necessary meanwhile keep the heart rate not less than60times of the patients always. On the basis of the conventional control group treatment,the treatment group plus the Jianxin Granule which take three times a day and one packet every time.The course of treatment is both28days. Observing the changes of plasma BNP、plasma monocyte TLR4、plasma TNF-a、6minutes walk test and TCM syndrome effect between before and after treatment.Results:1、Clinical efficacy:Comparison of the TCM syndrome integral:the total effective rates of tcm syndrome of the control guoup is86%,the treatment group is92%,and the results between two groups have difference in statistics(P<0.05); Comparison of the6minutes walk test: after trentment the walk distance of both treatment group and control group are lengthened,and the results between before and after treatment both have difference in statistics(P<0.05),and the walk distance of treatment group lengthened significantly more than control group(P<0.05).2、Biochemical targets:the levels of plasma BNP、plasma monocyte TLR4、plasma TNF-a are all decreased compared with before treatment of both treatment group and control group, and all of the results between before and after treatment have difference in statistics(P<0.05),and the detection indexs of treatment group are all decreased significantly more than control group(P<0.05).Conclusions:Jianxin Granule can improve the heart function and clinic symptoms of the patients who belong to deficiency of Heart Qi、Blood and Water Stasis syndrome type chronic heart failure. The mechanism is related to that Jianxin Granule can suppress the expression of TLR4and decrease the inflammatory factor TNF-a release.
