| ObjectiveIn this experiment,Jianxin Decoction was used to treat chronic heart failure patients with Qi deficiency and blood stasis syndrome,to evaluate the clinical efficacy and safety,and to explore its possible mechanism of action.MethodFrom January 2018 to December 2018,60 patients with chronic heart failure(CHF)who were admitted to the outpatient and inpatient department of cardiovascular medicine of Anhui Hospital of Traditional Chinese Medicine were randomly divided into control group and treatment group with 30 cases in each group.The control group was treated with routine western medicine for heart failure.On the basis of the control group,the treatment group was treated with Jianxin Decoction once a day,twice a half hour after breakfast and dinner.Both groups were treated for 4 weeks.The changes of NYHA cardiac function grade,TCM syndrome score,quality of life score(QOLS),6-minute walking distance(6MWD),B-type brain natriuretic peptide(BNP),left ventricular end-diastolic diameter(LVEDd),left ventricular ejection fraction(LVEF),adiponectin(APN),leptin(LP)and other related indexes were observed before and after treatment,and the adverse reactions during the experiment were recorded.Result(1)TCM syndromes: There was no difference in TCM syndromes scores between the two groups before treatment(P > 0.05);after treatment,TCM syndromes scores in the treatment group were significantly lower than those in the control group(P < 0.05);meanwhile,for patients with single TCM syndromes,both groups improved after treatment,but the treatment group had more advantages than the control group in improving single TCM syndromes,especially in short breath.The curative effect was more significant(P < 0.05)in respects of asthma,hypouria and so on.(2)Heart function effect: The total effective rate of improving heart function in the treatment group was 87.0%,while that in the control group was 60.0%.There was significant difference between the two groups(P < 0.05).The improvement of cardiac function in the treatment group was better than that in the control group(P < 0.05).(3)QOLS in Minnesota: There was no significant difference in QOLS between the two groups before treatment(P > 0.05);QOLS in both groups decreased significantly after treatment(P < 0.05),and the improvement of quality of life in the treatment group was more significant(P < 0.05).(4)6MWD: Before treatment,the 6MWD of the two groups was comparable(P > 0.05);after treatment,the 6MWD of the two groups increased significantly(P < 0.05)compared with before treatment,and the treatment group improved the life tolerance of CHF patients better(P < 0.05).(5)BNP,APN,LP indicators: before treatment,there was no difference in the biochemical indicators between the two groups(P > 0.05);after treatment,the above indicators of the two groups were significantly lower than before treatment(P < 0.05),and the treatment group was significantly lower(P < 0.05).(6)LVEF and LVEDd: Before treatment,there was no significant abnormality in the cardiac color Doppler ultrasound indexes of the two groups(P > 0.05);after treatment,the LVEF values of the two groups were increased(P < 0.05),but there was no significant difference between the treatment group and the control group(P > 0.05).At the same time,the LVEDd of the two groups after treatment was not significantly improved compared with that before treatment(P > 0.05),and there was no statistical significance.(7)No adverse reactions and side effects were found in both groups during and after treatment,and the clinical application was safe.ConclusionOn the basis of conventional western medicine anti-heart failure treatment,Jianxin Decoction can significantly improve the symptoms,TCM syndromes and cardiac function of patients with chronic heart failure with Qi deficiency and blood stasis syndrome,improve the exercise tolerance and quality of life of patients,and also reduce the levels of BNP,APN and LP in patients’ serum,which is superior to conventional western medicine anti-heart failure treatment alone.There was no significant difference in the improvement of LVEF between the two groups.Jianxin Decoction has no adverse events in the treatment of chronic heart failure of Qi deficiency and blood stasis type,and is safe in clinical application. |
