Objective:The application Peplau interpersonal theory through home visits by community elderly type2diabetic patients, elderly patients with type2diabetes, and the community to the implementation ofindividualized health education, to enhance the effect of home visits, home visits to scientific and practical,through home visits by community nurses provide reference; level of knowing and improving the health ofelderly patients with diabetes to reduce and delay the complications of diabetes, and to improve the qualityof life of patients; improve health education capacity for home visits by nurses, higher service satisfaction.Method: By using the method of typical sampling, according to the distribution of the proportion of elderlypopulation, chronic disease prevalence and andsocial economic distribution of Shihezi City,5,21,42district as a sample of community, with60years of age or older with type2diabetes and "home visitingnurses" as the research object,98elderly diabetics were random divided into intervention group and controlgroup, for intervention group carrying out family focus interview based on the four stages applicationPeplau interpersonal theory, and control group didn’t do any intervention study,through literature analysis,interviews and community test methods of presenting the community elderly patients with diabetes relatedknowledge, behavior, the control of blood glucose and the "family visiting nurses" abilities, etal, before andafter the family visit to form evaluation of Family Questionnaire visiting effect. Take statistical description;measurement data (x±s), using t test; counting the number of cases (percentage), using χ2test, test level(P<0.05)Result:1Orientation evaluation:8cases out of the intervention process exit,3patients in interventiongroup,5cases in the control group. Baseline data were higher in the intervention group: group of patientswith diabetes mellitus with hypertension, with significant difference (P<0.05), there were no significantdifferences in other relevant knowledge indicators in general, diabetic cognitive level, health (compliance)behavior, the control of blood glucose index (P>0.05), with comparable. At the same time displaycognitive level in diabetic patients with related knowledge was low, health (compliance) behavior was poor,poor glucose control.2Identification the common goal setting reflects the special suitability: Patients can reach70%ofdiabetes-related knowledge overall cognitive level and the overall level of health (Compliance doctors)behavior; the average level of not more than7mmol/l of fasting blood glucose, postprandial2hour bloodglucose level does not exceed10mmol/l (Asia-Pacific policy type2diabetes control target).3Resolution patients with growth evaluation: Intervention group patients in the home visits before andafter the self-control comparisons, and compared with control groups after home visits showed:interventiongroup was superior control group in the cognitive level of diabetes-related knowledge, health (compliancedoctors) behavior and glycemic control, the differences were statistically significant (P<0.05).Intervention group improved in patients with diabetes-related knowledge cognitive level, diabetes-related health (Compliance doctors) behavior change was obvious, get better control blood sugar. But nosignificant changes in blood pressure and body mass index differences (p>0.05). After the home visit, thepatients of the control group, the level of awareness of diabetes-related knowledge, health (Compliancedoctors) behavior changed, but there was no significant difference (p>0.05)4"home visiting nurses" growth evaluation: before and after family visit, health education ability of"homevisiting nurses" score comparison, the difference was statistically significant (P<0.05), health educationability of "home visiting nurses" was greatly improved.5The nurse-patient relationship evaluation: intervention group46patients on the table18(100%) entries ofthe satisfaction survey project satisfaction average score>4(satisfied), of which13(72.2%) entry averagesatisfaction score of5points (very good), family visit satisfaction is relatively high, at the same time theinterview also showed good nurse-patient relationship.Conclusion:1. the study tested the value of Peplau’s interpersonal theory is an important in home visits.2. Elderly Patients with diabetes home visiting based on Peplau’s interpersonal relations model, the effect isSignificant.3. Elderly Patients with diabetes home visiting based on Peplau’s interpersonal relations model, family visitsatisfaction is relatively high.4.
