Research BackgroundThe aging tendency of population shows current situation of the increasing of the disabled elderly people, elderly people, empty nest elderly and elderly people with chronic diseases, and the number of pension institutions and beds can’t adapt to the aging population growth. Therefore, in order to cope with the aging of the population, and to safeguard and improve people’s livelihood, in October15,2013the State Council promulgated "Several opinions on accelerating the development of pension services", which explicitly points out, by2020, our country will build the system of elderly care service, which are based in home and community, supported by institutions, having perfect function and appropriate scale, covering urban and rural. It will put life care, medical care, spiritual comfort and emergency rescue coverage of all the elderly at home. As the core element of home care, the development of the elderly medical and nursing care is slow, compared to other pension industry. The traditional medical model "to the hospital, outpatient follow-up" is not only more and more difficult to meet the needs of the elderly demand for health care, but also can’t effectively achieve the important organ disease advance warning, early detection, timely treatment and can’t avoid sudden death. To solve these problems, we need change the existing passive mode "attack to the hospital" to modern medical model "early warning and early active treatment".With continuous application of big data, cloud technology, networking and other information technology in the field of medical and health, a lot of health products and equipment appear, such as intelligent bracelet, electronic blood glucose meter, health APPs and so on, which make the coverage of medical health service extend to family, in order to realize Tele-home health management, through achieving intelligent data on human. The prevalence rate of diabetes was ranked third. And diabetes emphasizes the management of patients outside hospital. Based on this, relying on diabetes health management in a hospital of Guangzhou, under the support of information technology such as the Internet of things and remote medical treatment in the hospital, the study build the Tele-health management model of elderly patients with diabetes based on Internet of things in a first class hospital. Based on the model, elderly patients with diabetes can measure blood sugar and blood pressure and obtain health assessment and guidance by endocrinologist in order to improve the consciousness and ability of Tele-home health management.Research Purpose1. For the elderly, with the help of the internet of things, telemedicine and self-care mode, patients with diabetes can monitor their own health at home and obtain health assessment and guidance by endocrinologist in order to change the behavior and life style actively and improve the health level; the model makes the patients at home enjoy professional, intelligent and convenient medical services, at the same time saving time and money.2. For the hospital, the model can create a flexible working time for the doctor and improve the treatment efficiency at the same time reducing phenomenon of big hospital overcrowding and queue and ease the problem of seeing a doctor difficultly and expensively.3. For the society, the model saves family medical expenses, reduces the excessive waste of social resources and improves the overall health. Research Methods1. Literature ResearchThe article collected, summarized and sorted out relevant literature information. Research was made based on literature research, which laid the theoretical foundation for research framework and thesis writing. Through literature analysis, the paper determined the research contents:study of Tele-health management model of elderly patients with diabetes based on Internet of things in a first class hospital. And this study determined the stakeholders influencing building the model, including elderly patients with diabetes, family members, endocrinologists, nurses in Department of Endocrinology, hospital managers and health information technicians.2. Semi-structured InterviewsInterview was for the purpose of stakeholders’demand analysis, and investigated their demand of Tele-home health management service based on Internet of things in elderly patients with diabetes, thus preliminarily determined the mode of the model’s operational process and service content, so as to provide the basis for the next phase of research.(1)Through interviews with elderly patients with diabetes, this study intended to know the patients’ health problems at the time of diabetes prevention, treatment and rehabilitation.(2)Through interviews with families, the paper interviewed difficulties of elderly diabetes patients’ families in the process of caring and nursing patients with diabetes at home.(3)Through interviews with doctors and nurses in Department of Endocrinology, this paper wanted to realize what kind of health management services doctors and nurses could provide, at the same time confirmed the work that need patients and families do during home and hospital.(4)On the interview of hospital managers, the paper aimed at understanding their opinions about Tele-home health management services based on the internet of things.(5)Through interviews of health information technicians, the study wanted to know whether the existing information technology could support the demands above.3. Questionnaire InvestigationWith the fill type questionnaire, this study investigated service content of Tele-home health management for elderly patients with diabetes based on internet of things in order to verify approval ratings of service content in large sample.The questionnaire included two parts:personal basic situation and demand investigation of Tele-home health management service for elderly patients with diabetes based on internet of things. Inclusion criteria of survey respondents were as follows:(1) the elderly aged60and above;(2)diabetic (3)accept questionnaire voluntarily;(4) Respondents have clear consciousness and can understand the content of the questionnaire.The survey was done under the agreement of the selected units and respondents and lasted from DEC.2013to Jan.2014. Finally, the effective sample size was186.4. Statistical AnalysisThe paper counted demographic variables data and the questionnaire items. According to the result of statistical analysis, the study determined the model’s service content and put forward countermeasures and suggestion in order to provide a basis for the construction of model.Research Results1. On the basis of semi-structured interviews, the study preliminarily determined the model’s operational process and service content.Model’s operational process:elderly patients with diabetes measure blood sugar and blood pressure at home and transfer data to doctors by health terminal (panel computer and smartphone). According to the index, the doctors evaluate patients’ health status and give health guidance. Patients change their own lifestyle and realize diabetes self-management under the doctors’advice.Model’s service content:self-monitoring, health assessment, health intervention, health reminder, health inquiry, health education, emergency call, hospital registration, check inspection report, health games, APPs download.2. According to the result of questionnaire, all entries’ approval ratings were higher than50%except health game and most of the entries’ approval ratings were above80%. It showed that186elderly patients with diabetes were certain of health management model’s service content.3. The statistical results of questionnaire survey showed that only health games’ approval ratings were lower than50%in19items. So the study deleted health games and confirmed the content of Tele-home health management for elderly patients with diabetes based on internet of things, which were self-monitoring, health assessment, health intervention, health reminder, health inquiry, health education, emergency call, hospital registration, check inspection report, APPs download.4. According to the result of questionnaire, entries of approval ratings hig her than90%were focused on health assessment (96.6%), health education(96.0%),health intervention (93.3%),health (92.4%) and health reminder (91.3%). It showed that elderly patients’ degree of demand with diabetes on these five aspects were relatively high.5. Innovative service content’s approval ratings such as APPs download, hospital registration, check inspection report were between70%to80%, and they were not high compared to that of health education and health inquiry. Through reviewing the respondents, the autor understood that patients were down of accepting the new things and doubted the practicability of service content, leading to only willing to accepting conventional Tele-home health management services.6. Though the combination of semi-structured interviews and questionnaire survey, the autor confirmed model’s service content and operational process. As the starting point, the study built Tele-health management model of elderly patients with diabetes based on Internet of things in a first class hospital by embedding the service content into the operational process. It included five links that were Tele-health monitoring, carrying out health assessment, developing h ealth intervention, carrying out health education and putting into health trackin g and visitor.Research ConclusionThe model of Tele-health management for elderly patients with diabetes based on Internet of things in a first class hospital is the effective bridge of combining doctors and patients and hospitals and home. Before building the model, the autor need to clear the operational process and service content of the model. This study adopted semi-structured interviews firstly for the purpose of analyzing stakeholders’ demand, and preliminarily determined the model’s operational process and service content. Through the questionnaire survey method, the study verified approval ratings of service content in large sample, and confirmed the service content of the model finally. The paper integrated the content of the model, put the content into the operational process and then built the model. According to the research needs, the study designed Tele-home health management system for elderly patients with diabetes based on the theory of model. By the system, elderly patients can experience Tele-home health management service and accomplish the process of health monitoring, health assessment, health intervention, health education and health tracking and visitor.
