A Research On Home Visiting Based On The Nursing Process With Community Elderly Patients Hypertensive

Objective: Home visitinging based on nursing process with community elderly hypertensive. Data werecollected from objective and subjective and is used assessing elderly hypertensive. Nursing diagnoses,nursing planning, nursing interventions and nursing evaluation were identified by standardized nursinglanguage. To evaluate the effectiveness of home visiting based on nursing process with community elderlyhypertensive.Method: Choosed120elderly hypertensive from three communities in Shi he zi, and randomly dividedthem into test group and contrast group. The patients in contrast group were conduct health education bycommunity health service worker. The patients in contrast group were carried on the above-mentionedtreatment and also home visiting by reseachers. The results were compared by Nursing Outcomes.Results:1.24nursing diagnoses,58nursing interventions and71expected nursing outcomes/nursing outcomeswere identified.2. Through home visiting, the function health, social and psychological health, health konwledge andbehavior, perception health, pressure value were better than before. but the physical health, BMI value intest group and contrast group had no marked difference.3. The function health, social and psychological health, health konwledge and behavior, perception health,pressure value in test group were better than that in contrast group. but the physical health, BMI value intest group and contrast group had no marked difference.Conclusion:1. Standardized nursing language is suitable for home visiting.2. Application of standardized nursing language standardization work content, embodies thehuman-centered home visiting3. Home visiting based on standardized nursing language reflects the health and effective managementmode, can improve the health level of the elderly patients with hypertension.4. Home visiting based on standardized nursing language, can enhance patient health belief, improvepatient health knowledge level, improve patient health behavior.5.
