Purpose:This study aims at knowing about advanced cancer patients'level of self-perceived burden and the centre of advanced cancer patients'self-perceived burden. And it studies how clinical nursing intervention influence on advanced cancer patients' self-perceived burden and each dimension self-perceived burden Level to explore the differences of the influence of patient's individual information on level of self-perceived burden.Methods:(1) Research subject:June 1,2009 to February 30,2010 in Changzhi City People's Hospital, the in-ward-patient treatment in line with the conditions included cancer patients as a continuous survey. Inclusion criteria:Patients who are a diagnosis of advanced cancer,18 years old or above, have no mental illness in the past and severe cognitive dysfunction, and with a high enough educational level can understand the content of questionnaires. (2) Research Methods:To measure by applying Self-Perceived Burden Scale. There are 10 entries, three dimensions in the Scale, including body burden, emotional burden, and economic burden. Using 1-5 rating system, it can measure patients'SPB after summation. The higher score means heavier burden. The patient disease information Questionnaire which is made, mainly includes the gender, age, Cancer site, the course, the family finances,payment of medical expenses, the main treatment modalities and the extent of their knowledge of disease and so on. During the investigation, by means of the nursing intervention, mainly includes health education, we can compare patients'Self-perceived burden before and after nursing intervention. (3) Data processing and statistical analysis:Using statistical software SPSS16.0 to make the statistical analysis of data. Specific analysis methods included descriptive statistical analysis, factor analysis, variance analysis and multiple regression analysis.Result:(1) 216 patients with advanced cancer experience a high self-perceived burden, including self-feeling the burden of the intervention group mean 40.57±7.68, feel the burden of self-control mean 38.98±8.45, more than 30 minutes of moderate burden of being the critical point. (2) Baseline evaluation, the intervention group and control group in the degree of self does not feel burden of significant differences (p> 0.05). One month after the second evaluation, the two groups in self-feeling the burden of a significant difference (p<0.05). (3) By adopting methods of health education, clinical care and intervention, can significantly reduce the burden on patients self-feelings. After intervention, the intervention group were feeling the burden of self-mean 36.33±6.15, although more than 30 minutes to moderate the burden of the critical point of self-feeling, but lower than before the intervention significantly reduced the burden of self-feelings. (4) The burdens of the patients with advanced cancer feel are mainly the burden of the main self-contained body burden, financial burden, emotional burden. Emotional burden of patients with advanced cancer is the most important of all the burden of the burden, followed by the economic burden and, finally, the body burden. Use of clinical nursing intervention in reducing the emotional burden of patients with advanced cancer are most effective. (5) The differences of information of individual patients with advanced cancer,such as the age, knowledge of the disease, family income, the model of medical expenses, payment affect the dimensions of SPB, but after nursing intervention, all of the factors for the impact of the various dimensions of burden has been reduced.Conclusions:(1) The terminal cancer sufferers have comparatively high self-perceived burden, which is affected by a number of individual factors of the sufferers. Therefore, in the care process of the terminal cancer sufferers. It is significant to have detailed information of the sufferers and do the care according for improving the care and relieving the sufferers' self-perceived burden. (2) By adopting health education method and the like to do the clinical care intervention treatment. We can lower the sufferers'mind stress remarkably. Therefore, in the treatment process, by adopting scientific and reasonable care methods, we can lower the sufferers'self-perceived burden. (3) The burdens of the patients with advanced cancer feel are mainly the burden of the main self-contained body burden, financial burden, emotional burden. Emotional burden of patients with advanced cancer is the most important of all the burden of the burden, followed by the economic burden and, finally, the body burden. Use of clinical nursing intervention in reducing the emotional burden of patients with advanced cancer are most effective.. (4) The individual information difference of the terminal cancer sufferers in the comparison group does affect different dimensional stress of the sufferers, however, in the intervention group, which goes through clinical care treatment, the effect on different dimensional stress by such factors as the age, the condition of knowing about illness, the family income condition, the payment ways of medical fees, is lowered accordingly. As a result, by adopting scientific clinical care methods. We can reduce the interference caused by the individual differences of the sufferers and lower the self-perceived burden of different levels effectively. Meanwhile, by doing so, we can improve the medical care to a greater extent and lower the effects on medical care by the individual differences of the sufferers.
