[Aim] The incidence of both diabetes mellitus and its chronic complications tends to increase and there is a curious phenomenon that the level of blood sugar is out of proportion to serious symptoms. The related laboratory check can only reflect the level of index being controled,but it can't estimate patients' whole conditions. Traditional Chinese Medicine has its peculiar character in preventing diabetes,relieving symptoms and postponing the occurrence of diabetic chronic complications. This study is one part of the significant project presided over by my tutor -"The TCM research on vascular complications of diabetes mellitus" spon-sore by Beijing Municipal Seience and Technology Commission. We attempted to assess the patients' quality of life with different courses of diseases,applying Diabetic Specific Quality of Life Profile(DQOL)and TCM Clinical Differentiation Profile,to search the correlation of the QOL and laboratory index and to establish a simple,perfect and effective method for clinical assessment ,which can not only reflect the thoughts of TCM theory,but also estimate the TCM clinical effects better.[Subjects] the number of the patients is 301,some patients are from Guang An Men Hospital the China Academy of TCM ,some are from Oriental Hospital affiliated to Beijing University of TCM and the others are from communities. The patients,whose courses of diseases is less than one year are 201 and the rest are 100,whose courses of diseases is between 5 and 10 years.[Methods] The patients' imformation which was as follows,lab index and two relative profiles was monitored every six months During this period,the patients took medicine regularly,adjusted the kind and dosage of medicine according to the development of disease and medical advice. At the same time, the outcomes of lab index and two relative profiles were recorded and put into database accurately, periodically and on time,and chose the imformation of the first and the 36th month as subjects. The imformation of the other 100 cases was chosen randomly from in-patient and out-patient departments.SPSS13.0 was adopted to analyze the relative data afer they had been entered and diaposaled. The main statistic methods contain descriptive analysis,paired-samples T test and multiple regression,et al. If P<0.05,the difference is considered significant.[Results] Among the 201 patients who have been chosen,the patients who have no symptoms of three "polys"and one little are more than the ones who have relative symptoms.49.8 % patients haven't got the relative symotoms,and 23.9% patients have no symptoms when they were diagnosed T2DM.The fact indicates that the level of blood sugar is out of proportion to serious symptoms,which is different from "Xiao Ke"written in TCM Internal Medicine;Compard the laboratory index of the first and the 36th by paired-samples T test,the latter is obviously lower than the former ;the longer the course of the disease is ,the worse the quality of life is. As far as every aspect of the QOL for the patients whose courses of disease are between 5 and 10 years is concerned, the scores from high to low (the higher the score is,the worse the QOL is )are Spirit / psychology, physiology, social relationship and therapy. TCM Clinical Differentiation Profile shows that the most serious influence to QOL is the symptoms related to diabetic chronic complications; There is no remarkable correlation between laboratory index and QOL,which proved that only using laboratory index to estimate the clinical effect is not enough.[Conclusions] Since the number of T2DM patients with "no obvious symptoms" is larger and larger,it is very important to screen the people who were suffering from T2DM,but they didn't perceive it,which can reflect the thoughts of TCM prevention. In the early stage of T2DM,the laboratory index will improve to some degree by proactive and multiple interventions .The longer the course of disease is ,the worse the quality of life is. The main factors influencing QOL of T2DM are chronic complications and Spirit / psychology,and the others are physiology, social relationship and therapy,that is to say, preventing chronic complications is equally important besides controling hyperglycimia. There is no remarkable correlation between laboratory index and QOL,which indicates that QOL should be considered during the course of diagnosis and treating T2DM as an important clinical assessing aspect.
