Objective:To investigate primary nursing knowledge, attitude and practice of clinical nurses among tertiary-A hospitals in Changsha, and explore its relevance, understand the impact factors among clinical nurses, so as to provide a theoretical basis to improve the level of clinical nurse'knowledge, attitude, practice, to provide a advice for the administrator.Methods:From Sep 2010 to Oct 2010, with a method of stratified cluster random sampling, questionnaires were distributed to 440 clinical nurses working in four public tertiary-A hospitals in Changsha (Xiangya Hospital, Hunan People's Hospital, Hunan Traditional Chinese Medicine Hospital, the First Hospital of Changsha City).The instruments included demographic data form, self-designed inventory of knowledge, attitudes and practice. SPSS 13.0 package was used for data analysis, statistical methods included descriptive statistics, t test, variance analysis, Chi-square test,rank sum test,correlation analysis and multiple linear regression analysis.Results:1. The mean score of primary nursing knowledge of clinical nurses in Changsha tertiary hospitals was 19.58±2.28,mean score counted 85.1%of total score. The mean score of primary nursing attitudes was 70.78±9.65, which counted 67.4%of total score. While the mean practice score of primary nursing was 207.14±25.18, 79.7%of the total points.2. Correlation analysis showed:clinical nurses'knowledge of primary nursing was positively correlated with attitudes(r=0.129,P<0.01),attitudes towards primary nursing was significantly positively correlated with practice (r=0.259,P<0.01),while the knowledge of primary nursing was positively correlated with practice (r=0.200,P <0.01).3.The knowledge scores of primary nursing is significantly different between nurses from different levels of tertiary-A hospitals in Changsha (P<0.01), the attitude and practice scores were not statistically significant.4.One-way ANOVA analysis showed:the KAP scores of primary nursing were significantly different among the pilot hospitals, clinical nurses'age, working years, job title, education, ward, average time spent on morning and evening routine nursing and daily position (P<0.05)5.Stepwise regression analysis (multiple linear regression) showed that:main factors impacting clinical nurses'primary nursing knowledge score were total score of basic nursing behavior, the views of primary nursing, working years, basic nursing skills condition; main factors impacting the primary nursing attitude were the basis for nurses are the main actors of primary care, job titles, nurses'duty, the concept of basic nursing, whether a pilot hospital; main factors impacting clinical nurses' primary nursing behavior is care will, knowledge needs, the first professional qualifications, average time spent on morning and evening routine nursing, total score of basic nursing knowledge.Conclusions:1. Of all the scores of primary nursing knowledge, attitude and practice among clinical nurses in Changsha tertiary-A hospitals, the knowledge score was relatively higher, while the score of practice was lower,and the score of attitude was lowest.2.Clinical nurses'primary nursing knowledge, attitudes and practice were positively correlated.3. The knowledge scores of primary nursing were significantly different between nurses from different levels of tertiary-A hospitals in Changsha, the attitude and practice scores were not statistically significant.4.The factors impacting clinical nurses'primary nursing knowledge, attitude and practice are related to working years, job title, the first professional qualifications, average time spent on morning and evening routine nursing, whether a pilot hospital.5. Clincal nurses'knowledge,attitude and practice on primary nursing influence each other.
