Objective(s):The multi-disciplinary collaborative continuity nursing intervention was applied to stroke patients discharged from hospital to observe the effect of this model on knowledge,attitude and practice of stroke patients;to verify the effect of multi-disciplinary collaborative continuity nursing;and to provide a reference for improving the future nursing service model of hospitals.Methods:1.From December 2018 to February 2020,204 stroke patients were selected from the Sixth Affiliated Hospital of Kunming Medical University by the method of continuous inclusion.When they were discharged from the hospital,they were investigated with basic data and questionnaire of stroke knowledge,attitude and practice to understand the basic situation of the patients.2.204 patients were randomly divided into experimental group(102 patients)and control group(102 patients).Multidisciplinary collaborative continuing care+routine discharge follow-up was adopted in the experimental group,while routine discharge follow-up was conducted in the control group3.The differences in knowledge,attitude and practice of stroke between the experimental group and the control group one year after discharge were compared and analyzed.Results:1.There was no significant difference between the two groups in gender,nationality,occupation,marriage,education,payment mode,admission department and other basic information(P>0.05);there was no significant difference in the past history of TIA,hypertension,diabetes(P>0.05);there was no significant difference in the lifestyle of smoking,drinking and sports(P>0.05)2.There was no significant difference between the two groups(P>0.05)in the overall score median of stroke health knowledge at the time of discharge,but after 1 year of discharge,the median scores of the experimental group and the control group were higher than those before intervention,and the difference was statistically significant(P<0.05);after 1 year of discharge,the overall median score of health knowledge in the experimental group was higher than that in the control group,The difference was significant statistically(P<0.05)3.There was no significant difference between the two groups(P>0.05)in the overall score median of stroke health attitude at the time of discharge,but after 1 year of discharge,the scores median of the experimental group and the control group was higher than that before intervention(P<0.05);after 1 year of discharge,the overall score median of health attitude in the experimental group was higher than that in the control group,The difference was statistically significant(P<0.05).4.The overall median score of stroke health practice at the time of discharge was no significant difference between the two groups(P>0.05),but after 1 year of discharge,the median scores of the two groups were higher than those before intervention(P<0.05);after 1 year of discharge,the overall median score of health practice in the control group was lower than that in the experimental group,with statistically significant differences(P<0.05).Conclusion(s):The multi-disciplinary cooperative continuous nursing is more effectively to strengthen the disease health knowledge of stroke patients,effectively improve the attitude of patients to overcome the disease,effectively change the bad practice style of patients,and plays a very important role in the prevention and treatment of stroke.
