Investigation Of Pre-hospital First Aid On Community Service Workers And Study On The Optimum Training Program

Objectives:For provide a basis of the development of community service workers' pre-hospital emergency training programs,we analysis community service workers' KAP(knowledge, attitude, practice) status of pre-hospital emergency care. The same time, according to survey results, combined with domestic and international status quo to explore the different pre-hospital emergency training programs for community service workers and the training effect in order to establish appropriate community service workers'pre-hospital emergency training programs.Methods:1. Survey:In September to October2008, we used convenient sampling method to select 146 community service workers in three communities(old Simon, the Bund, semi-Sung garden)of ShangHai to conduct random surveys. After the investigation, according community service workers whether had received training to divided them into three groups, the workers which never received training (n=66), received once training (n=39 people), received twice or more times training (n=39).2. Intervention evaluation:In May 2009, we used convenient sampling method to select 91 community service workers in the old Simon and the Bund communities, then according the law of random number to divide the workers into three groups.Group A (n=31):lecture method→demonstration method→exercising method→AssessmentGroup B (n=30):lecture method after playing DVD→emonstration method→exercising method→AssessmentGroup C (n=30):lecture method after playing DVD→demonstration method exercising method→simulated teaching method→AssessmentOn the time before training, the time immediately after training, a month after training,3 months after training, we used the Red Cross's scoring criteria to determine the best training programs and analyze the impacts of training factors. The above data were inputted through the Excel software, then using SPSS statistical software to analysis.Results:1.Survey:(1)General situation:Three groups had no significant difference (P<0.05) in general situation.(2)knowledge:Three groups were good commanding of the knowledge and skills of how to handle of the common pre-hospital emergency situations, such as the proper handling of electric shock, gas poisoning, etc. and their correct rate were more than 70%.But when come to which communication can be used for pre-hospital emergency help and the common pre-hospital emergency skills (cardiopulmonary resuscitation, bleeding, bandaging, fixed, moving),the three groups'correct rate were less than 50%.(3)Attitude:①The community service workers selected the more urgent and very urgent in demanding of handling of the common pre-hospital emergency situations and pre-hospital emergency skills were more than 77.1%.The community service workers selected necessary and very necessary at grasping the common emergency knowledge and skills was 85.4%.94.4% community service workers accepted that the pre-hospital training had positive effects to more or entire community.②Three groups in demanding of training time, training teacher, training form had no statistical difference. (4)Practice:①In case of emergency, the choice of three groups showed statistically significant (P<0.05).And the number showed that the more training the workers had received, the more they likely to be taken first aid as their first behavior.②Excluding the training factors, the sources of emergency knowledge of three groups was not statistically significant (P>0.05).2. Intervention evaluation:①Before training, the general situation of three groups was not statistically significant (P>0.05).②Before training, the results of assessment of knowledge and skills was not, but the results of knowledge was higher than the skills results.③Repeated measures analysis of variance showed that different grouped after using different training methods, except cardio-pulmonary resuscitation and bandaging showed no significant difference (P>0.05), others assessment projects all showed statistically significant difference (P<0.05). At the same time analysis also showed different time's training results were statistically different (P<0.05).④At each time analysis of variance showed that the time immediately after training and 1 month after training, three groups' results were not statistically significant (P>0.05), but when the 3 months after training the three groups' results showing a statistically significant difference.⑤sing the overall average of the time immediately after training, a month after training,3 months after training to be the dependent variable to do multiple linear regression analysis, the results showed that the variables of management, education, group B, group C, the overall average of the time immediately after training and 1 month after training had relevant with the dependent variable.Conclusions:1. The Community service workers can better grasp the handling common pre-hospital emergency situations; for the knowledge of common pre-hospital emergency, their answer showed the state of polarization(they can good command of part of knowledge; but others were not);they can not good grasp the skills of pre-hospital emergency.2. Community Services workers have training needs and wishes.3. Using 16-hourr as their certified first responder's training time; creating a unified standard in order to build pre-hospital emergency training system in China.4. Regulate the establishment of the team of pre-hospital emergency ladder training teachers.5. Pre-hospital first aid training can effectively improve the community service pre-hospital first-aid knowledge and skills, and Group C's training program is the best for the community service workers.6. In order to reduce the impact of forgetting, it is recommended re-training the cardiopulmonary resuscitation on the workers in 1 month after the training; and re-training all knowledge and skills in 3minth after training.7.The Community service workers' pre-hospital emergency training is being affected by many factors, including trainee's own educational level, whether trainee is the manager, training methods; of these when trainee were the managers would be available to play an active and leading role, so we can used it in training.
