Purpose:To observe the clinical efficacy of the combination of traditional Chinese medicine and western medicine treatment joint enhanced external counter pulsation to treat unstable angina(syndromes of phlegm and blood stasis) and the effects to the levels of NO and ET-1.To verify the treatment effect of the enhanced external counter pulsation, to investigate the mechanism of enhanced external counter pulsation.Material and method:We choose 44 hospitalized patients that hospitalized in the department of Cardiology in Affiliated Hospital of Liaoning University of Traditional Chinese Medicine who consistent with unstable angina(syndromes of phlegm and blood stasis).These patients were randomly divided into treatment group and control group, each group of 22 people, patients in the treatment group on the basis treatment of conventional western medicine and the decoction of Wen Ban Soup combined with EECP therapy, once a day,every 60 minutes,35 hours for a course, patients in the control group on the treatment of conventional western medicine and Wen Ban Soup. Both groups were blood before the first EECP and after the course of EECP, the blood was used to determine the level of NO and ET-1. Angina episodes and traditional Chinese medicine symptoms change were recorded before and after the treatment.Results: 1. On anti-angina pectoris aspect, total effective rate in treatment group was 95.5%, in the control group it was 86.4%, there was no statistically significant difference(P>0.05); however on two remarkable efficiency, the former is 54.5% and the latter was 22.7%, it means there were significant differences(P <0.05).2. Before treatment,total score of TCM Syndrome between two groups has no statistical difference(P>0.05); In control group, total score of TCM Syndrome were significantly different before and after treatment(P <0.01); In treatment group, total score of TCM Syndrome were significantly different before and after treatment(P <0.01); After treatment, total score of TCM Syndrome between two groups were significantly different(P <0.05). 3. On aspect of the efficacy of TCM syndrome, total effective rate in treatment group was 95.5%, in the control group it was 90.9%, there was no statistically significant difference(P>0.05); However on two remarkable efficiency, the former is 50.0% and the latter was 18.2%, it means there were significant differences(P <0.05). 4.NO in experimental group(EECP group) were significantly higher after treatment, compared before and after treatment was statistically significant(P <0.05);After treatment NO levels in the control group also increased, and the contrast before and after the experiment was statistically significant(P<0.05); NO levels after treatment in experimental group was higher than that of control group,the contrast was statistically significant(P <0.05).ET-1 in experimental group were significantly lower after treatment, compared before and after treatment was statistically significant(P <0.05); After treatment ET-1 levels in the control group also decreased, and the contrast before and after the experiment was statistically significant(P <0.05); ET-1 levels after treatment in experimental group was lower than that of control group,the contrast was statistically significant(P <0.05).Conclusion: 1.Enhanced external counterpulsation therapy combined with Traditional Chinese Medicine can improve the symptoms of angina, TCM symptoms and relatively inexpensive, which is worthy of promotion to be applied to unstable angina(phlegm syndrome) 2. Its efficacy may be related to increasing nitric oxide(NO) expression levels,reducing endothelin(ET-1) expression levels.
