Objective: Observation of external counterpulsation with combining traditional Chinese and western medicine therapy in patients with coronary heart disease(CHD)PCI postoperative blood stasis in the clinical curative effect and the influence of related parameters of cardiopulmonary exercise test,to investigate the plausible mechanism of the combination of external counterpulsation combined with traditional and western medicine on the improvement of blood stasis syndrome after PCI,and improve the prognosis and enhance the long term efficacy,to offer reference material for clinical therapeutic scheme.Methods: 60 patients undergoing PCI after treatment with coronary heart disease admitted in the Department of Cardiology,Henan Provincial Hospital of Traditional Chinese Medicine from November 2018 to January 2020 were randomly divided into control and treatment group of all 30 patients.The control group was treated with xuefu decoction plus prescription and the primary drug treatment,while the treatment group was used by similar measures and extracorporeal counterpulse therapy.The treatment cycle was 14 days.The basic data were observed and recorded in each group patients: age,sex,body mass index(BMI),smoking status,residual stenosis and associated diseases include hypertension,diabetes,and hyperlipidemia.Records and statistics the test index in patients with cardiopulmonary exercise before and after treatment in two group: the peak oxygen uptake(peak VO2),anaerobic threshold(AT),peak oxygen pulse(peak VO2 / HR),metabolic equivalents(METs),carbon dioxide ventilatory equivalent slope(VE/VCO2)slope and quantification of blood stasis syndrome integral,the occurrence of adverse reactions during the treatment were compared in each group,monitor and compare the two groups in one week before and after treatment the blood routine,liver function,renal function,blood coagulation function and myocardial enzymes such as safety index.Excel was used to establish the database,and SPSS21.0 statistical software was used for data processing.The confidence interval was set at 95%,which had statistically significant.Results:1.General information: there was no statistically significant difference in the ag e,sex,body mass index(BMI),smoking status and residual stenosis between th e two groups(P > 0.05).Both in combined diseases: hypertension,diabetes,hy perlipidemia(P > 0.05).2.Comparison of cardiopulmonary exercise test indicators: peak VO2,anaerobic threshold(AT),peak VO2/HR,and METs,those values,before treatment,had no statistical difference between the two groups(P > 0.05).Both of them showed significant improvement after treatment compared with before,which was a statistical difference(P < 0.05).After treatment,the medical group was dramatically higher than the control group,and this distinction was not statistically significant(P < 0.05).The VE/VCO2 equivalent slope of the two groups had decreased at the before and after treatment,but there was no statistical difference(P > 0.05).3.Quantization score of blood stasis syndrome: after treatment,both groups had significantly lower quantization score than before,these condition had statistical significance(P < 0.05),while the quantization score of the treatment group was significantly lower than that of the control group after treatment,had statistical significance too(P < 0.05).4.Scores of main and secondary symptoms of blood stasis syndrome: the treatment group was superior to the control group in the improvement of main symptoms which included chest pain,chest tightness,palpitation and secondary symptoms: sublingual vein,tongue quality and pulse astringency(P < 0.05).However,there was no statistical significance in the changes of skin nail fault and lips and gums(P > 0.05).5.Curative effect of blood stasis syndrome: the total rate of the treatment group was93.33%(28/30),among all patients 9 cases(30.00%)showed obvious efficacy,19cases(63.33%)were effective,and 2 cases(6.67%)were ineffective.The total effective rate of the control group was 89.66%(26/29),including 5 cases(17.24%)with obvious effect,21 cases(72.41%)with effective effect,and 3 cases(10.34%)with ineffective effect.The treatment group was superior to the control group(P <0.05).6.Safety indicators: no serious adverse reactions occurred during the treatment(P >0.05);The blood routine,liver function,renal function,coagulation function and myocardial enzyme spectrum within one week before and after the observation were all within the clinical controllable range,and no obvious abnormalities were found.Conclusion:1.The combination of traditional Chinese and western medicine and xuefu zhuyu decoction combined with western medicine has an obvious effect on improving the blood stasis syndrome and cardiopulmonary function of patients after PCI.2.Extracorporeal counterpulsation combined with integrated traditional Chinese and western medicine can effectively improve the main clinical symptoms and some secondary symptoms of blood stasis syndrome in patients with coronary heart disease after PCI,promote the TCM syndromes,and improve their quality of life.3.The treatment of extracorporeal counterpulsation combined with integrated traditional Chinese and western medicine can enhanced the cardiopulmonary function and exercise tolerance of patients with coronary heart disease after PCI.4.The treatment method of external counterpulsation combined with Chinese and western medicine is safe and effective in clinical application.
