| Essential Hypertension (EH) is a kind of frequent and common disease with the principal symptom of heightening of systemic arterial pressure (SAP). This research aims to deeply explore the cause and mechanism, principle and method of therapy of hypertension from three aspects of theory, clinical literature files and experiment and to find the effective prescriptions for hypertension.This research regards the constitution as the inherent base of hypertension and puts forward the hypothesis of pathogenetic mechanism of intense fire in heart and liver, evil heat produced in the interior. Presume the therapeutic principle of purging liver and tranquilizing heat. The first part of this article, Systematic Review of Scientificalness of Pathogenetic Mechanism of Intense Fire In Heart and Liver, Evil Heat Produced in the Interior, based on evidence-based medicine, made a construction and a validation for both the pathogenetic hypothesis and the internal mechanism of corresponding therapy. Objective:systematic review of the efficacy and safety of treatment of hypertension based on the pathogenetic mechanism of intense fire in heart and liver, evil heat produced in the interior. Methods:Stipulate a highly sensitive search strategy, make the comprehensive retrieval of Chinese medicine literature of clinical control study of hypertension based on pathogenetic mechanism of intense fire in heart and liver, evil heat produced in the interior that up to September 2009, and make a Meta analysis of overall effect of a number of research results and conduct sensitivity analysis by using RevMan 4.2 software, and make an analysis of publication bias by funnel graph sheets. Results:Ten items of studies are finally included in this study,496 cases of patients were involved. Two of these 10 studies with test score of 3 points are high-quality literature, and the rest are low-quality literature. Meta analysis of combined effects of antihypertensive efficacy evaluation of the amount of RR (random) and 95% confidence interval (CI) 1.19 (1.10,1.29), Evaluation of combined effects of traditional Chinese medicine symptoms of the amount of RR (random) and 95% confidence interval (CI) 1.26 (1.14,1.40), safety evaluation of the integration effect of the amount of RR (random) and 95% confidence interval (CI) 0.24 (0.09,0.63), adverse reactions were mild nausea mainly occurred rate of 2.352%. Conclusion:The therapy of hypertension, based on pathogenetic mechanism of intense fire in heart and liver, evil heat produced in the interior, has somewhat advantage on the aespect of lowering blood pressure and improving the clinical symptoms, and be of fewer adverse reactions. However, since the quality of present methodology of trials is generally low and variability of tested compound prescriptions are large, there is not sufficient evidence to support its therapeutic applications. It requires much more high-quality randomized controlled trials.This research is based on the prior topic of Shandong provincial Science and Technology Department manufacture of capsule of dismissing heat and lowering blood pressure (serial number of authorization:Shandong provincial Science and Technology Department 2005)Number.103). Animal experiment indicates: NCCDHLBP can lower the blood pressure of both spontaneous hypertensive rats (SHR) and Hypertensive Rat Model (HRM) with obvious time-effect relation. It also can lower the level of plasma ET-1 and plasma vasopression II, inhibit the activity of RAS system. The research results have proved that the intense fire in heart and liver, evil heat produced inside is the key mechanism of EH. And purging liver fire and tranquilizing heart, fire-reducing and detoxifying is the important treatment of hypertension. NCCDHLBP has sound pharmaceutical and pharmacological bases through focusing many taches and target points. It is of integrated therapeutic effects and is one effective compound formular. According to relating statistics, there are nearly 200 million people have the disease of EH in our nation. There are still great challenges confronting us in the aspect of prevention and treatment of EH. There are so many rich clinical experiences and quite instructive exploration in the research of EH in Traditional Chinese Medicine. And this research deepened the cognition of mechanism of EH, provided a new thought of treatment of EH, and made a beneficial attempt for riching the pathology and therapeutics of EH.
