[Research background]Professor Niu Xingdong is the guidance teacher and Chinese medicine expert. He graduated from the Chinese medicine Department of Inner Mongolia Medical College, works in clinical, research of traditional Chinese medicine for more than 50 years. Professor Niu accumulates lots of experiments in traditional Chinese medicine, is specialized in treating spleen and stomach diseases, particularly in treating diseases such as Barrett’s esophagus and Diarrhea-predominant Irritable Bowel Syndrome etc. His clinic quantity has amounted to more than 20,000 annually. Many patients who suffer from difficult diseases have recovered through his treatment. His academic ideas has had great influence in Inner Mongolia and surrounding regions. Through years of theoretical study and practice, professor Niu has accumulated lots of theories and experiences. Many of his proved recipes, academic ideas and opinions need to be sorted out and developed with modern technological means by his students, which is also the entry point to combine traditional medicine and western medicine. Therefore, as his students, it is our duty to sort out, inherit, develop his clinical experiences and academic ideas so as to achieve theoretical improvement and summarize its’internal rules.[Research purpose]Through theoretical study and practice under teacher’s guidance, I’ve learned and sorted out professor Niu’s academic ideas and clinical experiences in treating digestive system diseases in the past 50 years. Particularly, I’ve focused on the research of his treatment of Diarrhea type irritable bowel syndrome.In this study, I attempts to develop and sort out professor Niu’s "theory of diarrhea by wind", finds out its effect and mechanism so as to apply those ideas and experiences in practice to improve treatment effect.[Research contents and methods]By joining in teacher’s outpatient clinic, attending his seminars, reading his works and studying his medical cases, I try to refine his clinical experiences. Through learning and retrieving ancient medical works, I’ve studied the classical theories of different times, refined and sorted out the origin and process of the forming of professor Niu’s academic idea of splenogastric diseases. Based on it, I’ve undertaken a deep study of these academic origins, summarized professor Niu’s classic academic ideas so as to enable those academic ideas to be widely used in clinic. By randomly selecting 75 cases of patients with diarrhea type irritable bowel syndrome, through a comparative study of clinical efficiency and inspection index between "Feng xie decoction" and "yi gong Powder", this study is carried out to verify the modern mechanism of professor Niu’s academic ideas.[Finding]1. The academic origin of Professor Niu XingdongThe beginning of this thesis is to discuss the classical theory of the spleen and stomach. They are the theoretical origin and the academic foundation of Niu Xingdong’s theory of the spleen and stomach.2.The second part of the paper is the specific discussion of the academic thought of Niu Xingdong’s stomach and spleen.With the experiences accumulated through joining in my teacher’s clinic in the past few years, under teacher’s careful guidance, I attempt to summarize my teacher’s experience about diseases of the spleen and stomach, refine tutor’s academic thought of the spleen and stomach. The main contents include:①Professor Niu has formed the academic idea of "the spleen and stomach should be conditioned firstly in treating diseases by traditional Chinese medicine," through years’ clinical experiences.②Professor Niu focuses on lifting and dropping activities of Qi in treating spleen and stomach in clinic, against just nourishing.③Professor Niu stresses that the five internal organs should be adjusted when we treat stomach diseases. Instead of limiting in the spleen and stomach, the relationship between the internal organs should be emphasized.④Professor Niu points out that the spleen and stomach can not be confused, which should be separated to treat.⑤Professor Niu pays attention to "dispel phlegm" in the treatment of stomach diseases.⑥When treating spleen and stomach disease, he pays attention to regulating qi and blood.⑦Professor Niu respects the ancients but has formed his own unique insights.He absorbs the knowledge of modern medicine, extends the Chinese medicine to a deeper level.3. The third part of the thesis is the clinical experience of treating the disease of the spleen and stomach with the guidance of the academic thought of my teacher.Professor Niu has formed unique methods in treating chronic atrophic gastritis, diarrhea type irritable bowel syndrome, reflux esophagitis, and functional constipation in the spleen and stomach disease.①A summary of the experience in treating Atrophic gastritis with Hp resistanceThrough Syndrome differentiation and treatment, Professor Niu adjusts the internal environment of the human body so as to make it unsuitable for the survival of Helicobacter pylori.② A summary of the experience in treating Barrett’s esophagusProfessor Niu believes that the main mechanism of Barrett’s esophagus is "the spleen weak, liver stagnation", phlegm and blood stasis are condensed each other, strong spleen and Condition liver by traditional Chinese medicine, get rid of phlegm and blood stasis by buccal tablet.③ A summary of the experience in treatment of constipationProfessor Niu believes that the wet constipation is a modern disease, adheres to adjust the Qi movement.④A summary of the experience in treatment of diarrhea type irritable bowel syndromeProfessor Niu makes it clear that the "wind evil invasion of intestine" is an important cause of diarrhea type irritable bowel syndrome. He puts forward the ideas of "strong spleen,dispel wind " in the treatment of D-IBS.⑤A summary of the experience of professor Niu Xingdong in the treatment of chronic insomniaProfessor Niu sums up the documentation and clinical experience by integrating TCM and modern research and finds the real reason:Yang can not enter yin.⑥A summary of the experience in treating children with cold:Cold and heat are expressed in quantity.⑦Professor Niu Xingdong uses "Dispelling wind medicine" in the treatment of D-IBS.4. Clinical researchObjective:To observe the effect of Feng xie Decoction on symptoms and on serum 5-hydroxytryptamine(5-HT) level in diarrhea-predominant irritable bowel syndrome (D-IBS).Methods:A prospective, randomize-controlled trial was conducted. Seventy-five Roma III positive D-IBS patients were equally randomized into three groups. The Feng xie group received Fengxie Decoction, the "Yi gong powder" group received "Yi gong powder" and the control group was treated with conventional western medicine.The course of treatment lasted 4 weeks. The clinical symptoms were observed and serum level of 5-HT was detected before and after treatment. Symptomatic scales, the SF-36 health survey scales and fecal flora were recorded before and after treatment.4 bacterials were cultured and the colony forming units were counted in stool. SPSS was used for data analysis. After treatment the clinical effect was evaluated. Results:Side-effect was not founded. The total effective rate was 100% in the Feng xie group, and was 76% in the control group, the difference being significant(P<0.05). Symptoms was improved, especially in abdominal pain and stool character. The scores of BP, GH, VT, SF, RE and MH scales had significant improved(P<0.05).The Feng xie Decoction could increase bifidobacterium count (P<0.05)and lactobacillus count(P<0.05);Serum 5-HT level in three groups after treatment differed from that before treatment (P<0.05), at the same time, the difference among the three groups after treatment was significant, too. (P<0.05).Conclusion:Feng xie Decoction is effective on relieving clinical symptoms and decreasing the expression of 5-HT in D-IBS patients. It can also significantly improve symptoms and intestinal bacterial flora, which is safe and effective in the treatment of patients with D-IBS.Research conclusion:The core academic thought of Professor Niu Xingdong is "the spleen and stomach should be conditioned firstly in treating diseases by traditional Chinese medicine," "Focusing on regulating qi and blood in treating spleen and stomach disease". He believes that an important cause of diarrhea type irritable bowel syndrome is"conceal the wind in the intestines".
