| Objective:By the study of effect of Buhuanjin Zhengqi Powder,adding the herbs of softening liver and smoothing Qi and the herbs of eliminating wind and coldness respectively,we probe the clinical characteristics of the Buhuanjin Zhengqi Powder which have a good effect on diarrhea-predominant irritable bowel syndrome(IBS-D)belonging to the syndrome of retention of coldness and dampness in the Spleen.From the study,we analyze mechanism of the herbs and conclude the key-point of syndrome differentiation of IBS-D which is caused by the accumulation of cokdness and dampness in the spleen and hope the result of the study could do help the treatment of IBS-D.Method:The total 90 outpatients were enlisted who visited the clinic department of gastroenterology of Jiangsu Province hospital of Chinese Medicine from March 2015 to February 2017.All cases are diagnosed IBS-D according to the Rome III diagnostic criteria whose syndrome is the syndrome of retention of cold and dampness in the Spleen at the same time.90 cases are randomly divided into three groups:treatment group one,treatment group two and controlled group with 30 cases of each group.The treatment group one is treated with Buhuanjin Zhengqi Powder adding herbs which can soften liver and smoothe Qi.The treatment group two is treated with Buhuanjin Zhengqi Powder adding herbs which can dispell wind and remove dampness.The controlled group is treated with BIFICO.The treatment continued 4 Weeks.The symptom scores before and after the treatment were recorded for analysis.Results:1 Overall clinical efficacy:The total effective rate was 93.33%in the treatment gro up 1 and 90.0%in the treatment group2.The total effective rate was 76.67%in the con trolled group.There was no significant difference in the total effective rate between the first treatment group and the second treatment group(P>0.05).The curative effect of the t reatment group 1 and 2 were better than that of the controlled group with significant dif ference(P<0.05).2 Clinical symptom scores:The changes of total scores of the three groups before a nd after treatment were statistically significant(P<0.01).There was no significant differen ce in total scores of symptoms remission between treatment group 1 and the treatment g roup2(P>0.05).The two treatment groups were better than the control group(P<0.05).3 The treatment group 1 was superior to the control group in improving abdominal pain,abdominal discomfort,abdominal pain after diarrhea,depression and irritability.Ther e were no significant difference between treatment group 1 and treatment group 2,treatm ent group 2 and controlled group in these aspects though the reduced symptoms scores of treatment group 1 were greater than that of the treatment group 2.4.The improvement of stool traits,feel more serious after cold diet,chills and cold symptoms,two treatment groups were better than the control group.There was no signi ficant difference between the two treatment groups in statistics.While the symptoms remi ssion degree of the treatment group 2 was better than that of the treatment group 1.5.All the three groups have a good improvement of following symptoms:the stool frequency,borborygmus and poor appetite.But there were no significant differences amon g the three groups in these symptoms.Conclusion:The curative effects of the two treatment groups were better than BIFI CO while the study shows no significant difference between the two treatment groups.R espectively,the treatment group added with herbs of softening liver and smoothing Qi ca n better improve the patient’s abdominal pain or abdominal discomfort,diarrhea after abd ominal pain,emotional depression and irritability symptoms.The treatment group added with herbs of dispelling wind and remove dampness can better improve the patient’s stoo 1 traits,feeling serious after cold diet,chills and cold symptoms. |
