Study On The Law Of Administration Of Diarrhea Type Irritable Bowel Syndrome

Objective:In this paper,we summarize the treatment of diarrhea type irritable bowel syndrome(IBS-D)in Treatise on Febrile Diseases and modern literature research,and analyze the characteristics of traditional Chinese medicine in the treatment of diarrhea type irritable bowel syndrome,so as to provide ideas and theoretical reference for the clinical treatment of this disease.Methods:1.Sort out the articles and prescriptions in Treatise on Febrile Diseases and synopsis of the golden chamber for the treatment of diarrhea type irritable bowel syndrome(abdominal pain and diarrhea),set up excel table,input the prescriptions and drugs,if the prescriptions contain the drugs,enter "1" in the cell,if not,enter "0",respectively sort out the drugs for the treatment of diarrhea and abdominal pain in Treatise on Febrile Diseases and synopsis of the golden chamber Frequency table,analysis of the top 10 drugs to draw a conclusion.2.With the help of CNKI platform,consult the literature about the treatment of diarrhea irritable bowel syndrome in the past 10 years,sort out the prescriptions that meet the inclusion criteria,input the prescriptions with Excel,the input method is the same as 1,and establish a database.In the same way,48 drugs which were used more than 10 times were treated twice,and their properties,taste and meridian tropism were sorted out.Cluster analysis was carried out to sort out the drug pairs,find out the correlation between the drugs and explore the compatibility relationship.At the same time,to explore the relationship between the treatment of diarrhea,abdominal pain and IBS-D in modern literature.Results:1.In Treatise on febrile diseases,there are 24 prescriptions for diarrhea,47 prescriptions,126 times in total;in Treatise on febrile diseases,there are 9 prescriptions for abdominal pain,21 prescriptions,48 times in total;in Synopsis of golden chamber,there are 11 prescriptions for diarrhea,23 prescriptions,39 times in total;in Synopsis of golden chamber,there are 7 prescriptions for abdominal pain,18 prescriptions,34 times in total.2.In the past 10 years,a total of 257 prescriptions of TCM in the treatment of diarrhea type irritable bowel syndrome were collected,121 prescriptions were used,and 2520 prescriptions were used.According to the descending order of the frequency of 121 drugs,it was found that 48 drugs were used more than 10 times,2247 times in total,accounting for 89% of the total frequency.The first 20 drugs were used more frequently than 30 times,the first 16 drugs were used more than 40 times,the first 8 drugs were used more frequently than 90 times(including 210 times of Atractylodes macrocephala,179 times of Glycyrrhiza,168 times of paeony,145 times of Poria cocos,143 times of tangerine peel,133 times of Dangshen,110 times of Fangfeng,94 times of Bupleurum).The most frequently used drugs were warm,mild and lukewarm,accounting for 77%;the most frequently used drugs were acrid,sweet and bitter,accounting for 85%;the most frequently used drugs were spleen,stomach,lung and liver meridian,accounting for70%.Cluster analysis showed 12 groups of drugs: 1.Fructus aurantii,ginseng;2.Tulip,cortex Albiziae;3.Xiangfu,Ligusticum chuanxiong;4.Shenqu,hawthorn;5.Huoxiang,Magnolia officinalis;6.Scutellaria,Pueraria;7.Jujube,ginger;8.Amomum,lentil;9.Lotus seed meat,Schisandra;10.Psoralea,nutmeg;11.Ginger,Coptis;12.Yam,coix seed.There are 5 clustering groups: Fang 1: Atractylodes macrocephala,liquorice,white peony,tangerine peel,Fangfeng,Poria cocos,dangshen;Fang 2: yam,coix seed,woody fragrance,dried ginger,Coptis chinensis,Fructus aurantii,Pinellia ternate,astragalus;Fang 3:nutmeg,Psoralea,Schisandra,Evodia rutaecarpa,aconite;Fang 4: lentil,lotus seed,Amomum,Platycodon;Fang 5: yanhusuo,angelica,ginger,jujube.3.Statistical analysis of the correlation between the treatment of diarrhea and abdominal pain in Treatise on Febrile Diseases and the treatment of IBS-D in modern literature shows that there are 54 drugs for diarrhea and abdominal pain in Treatise on febrile diseases,121 drugs for IBS-D in modern literature,and 31 drugs for both.Among them,more than half of the drugs are warm drugs;15 sweet drugs,14 pungent drugs;18 drugs return to the spleen channel,17 drugs return to the stomach channel,and 12 drugs return to the spleen and stomach channel at the same time.In addition,in the treatment of IBS-D in modern literature,nutmeg,Amomum villosum,gallnut,pomegranate peel and other astringent antidiarrheal drugs are used more than those in Treatise on febrile diseases.Conclusions:1.The treatment of IBS-D in modern literature is based on the treatment of diarrhea and abdominal pain in Treatise on typhoid and miscellaneous diseases,which should be added and subtracted mainly by traditional Chinese medicine.2.Based on the literature research,the law of compatibility of traditional Chinese medicine in the treatment of diarrhea type irritable bowel syndrome is proposed(1)Good use of spleen and stomach meridian medicine to take care of the nature of the day after tomorrow.Commonly used drugs for spleen and stomach: Atractylodes macrocephala,liquorice,white peony,Poria cocos,tangerine peel,dangshen,Fangfeng,etc.(2)Soothing the liver and relieving depression,restraining the wood and supporting the soil.Commonly used drugs: bupleurum,white peony,Fangfeng,Atractylodes macrocephala,Poria cocos,licorice,etc.(3)Heat the kidney and warm the spleen.Commonly used drugs: nutmeg,Psoralea,Evodia,aconite,Ramulus Cinnamomi,ginger,etc.(4)Astringent intestines stop diarrhea and relieve pain.Commonly used drugs: nutmeg,gallnut,Amomum,pomegranate peel,jujube,licorice,etc.3.We should pay attention to the processing of traditional Chinese medicine in clinical use.For example: salt roasted psoralen and stewed nutmeg can enhance the antidiarrheal effect;Coptis and ginger can retain its strong antidiarrheal effect,and can reduce the inhibitory effect of raw Coptis on gastrointestinal motility;vinegar Schisandra astringent essence has stronger antidiarrheal effect;fried lotus seed meat is used for spleen deficiency diarrhea;the antidiarrheal function of stewed Pueraria is stronger than that of raw Pueraria;wine Scutellaria can alleviate the bitter cold of raw Scutellaria and avoid diarrhea.
