| In order to change the long-term management status quo of medical treatment,medical insurance and medicine,we will continue to deepen the reform of the medical and health system.Since 2019,the National Healthcare Security Administration has led the organization to carry out the first batch of centralized drug procurement(hereinafter referred to as "national procurement"),which officially means that centralized drug procurement has become a national strategy.As a new purchasing mechanism for the transformation of medical insurance purchasing,the implementation speed and implementation effect of centralized purchasing with quantity of drugs are far more than expected.At present,the country has realized the double expansion from the city to the variety.At the same time,The General Office of the State Council issued the Opinions on Promoting the Regular and Systematic Implementation of Centralized Procurement of Drugs by Volume,putting forward suggestions to encourage local organizations to carry out centralized procurement by volume at different levels.At present,local organizations,especially provincial and trans-regional alliances,are also actively carrying out bulk procurement.However,in the process of policy implementation,medical institutions,medical insurance departments and pharmaceutical companies,the most active governance three parties,still have problems in the implementation of the policy.Based on practical problems and following the theory of drug supply and management,this paper sorted out the policy flow and implementation flow in the implementation mechanism of drug centralized procurement with volume.The research object is the coordination of the major and most active medical insurance department,medical institutions and pharmaceutical enterprise management in the normal development mode of Shanxi Province.First of all,the feasibility of the implementation of centralized drug procurement in Shanxi Province was analyzed from the synergy of governance objectives and the perfection of governance subject restriction mechanism.Secondly,the development results obtained under the normal development mode of classification and classification formed after Shanxi Province stepped into the normal development stage were summarized in stages.Finally,the paper analyzes the constraints and reasons that restrict the normalization process in the actual implementation process and explores the solution path.The research believes that there are some problems in the implementation of centralized drug procurement policy in Shanxi Province,such as imperfect policy formulation at the end of policy flow,supervision and punishment mechanism and poor communication mechanism between departments.At the implementation end,there are also problems such as insufficient governance ability,insufficient linkage of governance subjects,poor coordination of emergency treatment and imperfect platform building function.Based on the current governance status,from the responsibilities of governance subjects and the dimension of linkage,governance suggestions were respectively put forward for the current normal development of medical insurance departments,pharmaceutical companies and medical institutions in Shanxi Province,in order to improve the normal implementation mechanism through continuous correction,and realize the governance goal of centralized drug procurement with quantity "no restricted area" as soon as possible. |
