In the context of the deepening population aging,the situation of elderly hypertension prevention and treatment in China has become increasingly serious.As an important venue for the prevention and treatment of hypertension in the elderly,the community has limited medical and health resources to meet the health needs of the increasingly large elderly hypertensive population.In addition,hypertension lasts for a long time,and patients need to carry out daily health care treatment at home for a long time,which puts corresponding requirements on patients’ self-management ability.The value of social work in helping people to help themselves is consistent with the concept of "emphasizing the patient’s personal strength to maintain health" in chronic disease self-management,and the intervention of social work can also relieve the pressure of community medical and health institutions to a certain extent.Therefore,it is feasible for social work to intervene in the self-management of hypertensive chronic diseases in the elderly at the community level.This study used the case study method to collect information about self-management of elderly hypertensive patients in Community C through literature research,questionnaire survey,interview method and participant observation method,to investigate the current situation of self-management ability of elderly hypertensive patients in Community C,to discover the problems of patients,to predict the needs of elderly hypertensive patients in Community C based on the problem analysis,to carry out a group under the guidance of the enrichment theory based on the needs analysis Based on the needs analysis,we conducted a group to improve the self-management ability of elderly hypertensive patients under the guidance of the enrichment theory,evaluated the whole group using a combination of qualitative and quantitative assessment methods,tested the service effect of the group work,and finally reviewed the whole group service process,summarized and condensed the practical experience of group work intervention under the guidance of the enrichment theory,enriched the practical materials of social work intervention in the field of chronic disease self-management,and made a contribution to the formation of a systematic intervention model for social work intervention in this field.This will contribute to the formation of a systematic intervention model in this field.The present study found that group work intervention guided by enrichment theory can effectively enhance the self-management ability of elderly hypertensive patients in the community.In the group,patients achieved personal empowerment to improve their personal ability;interpersonal empowerment to improve their social ability;and social empowerment to accelerate social integration,based on the realization of the three aspects of empowerment,to solve the problem of insufficient self-management ability,including insufficient knowledge of hypertension and lack of attention to hypertension;insufficient motivation of self-management behavior and lack of scientific behavior management methods;cognition of available resources for disease management Insufficient knowledge of hypertension and lack of attention to hypertension;insufficient motivation of self-management behaviors and lack of scientific behavioral management methods;insufficient awareness of available resources for disease management and low utilization of resources,and finally to achieve the purpose of self-management capacity enhancement of hypertension.In the process of exploration,it was found that the group members’ mutual supervision helped to develop good behavioral habits,the improvement of the informal support network helped to enhance the motivation of the group members to change their behavior,and the social workers’ knowledge of good hypertension health care helped to improve the efficiency of the group.At the same time,there are still some shortcomings in the research design and practice design of this study,which need to be further improved and researched.
