| Purpose: Using data mining technology to identify the medication rules and academic ideas of Professor Zhang Mingxue who use the traditional Chinese medicine theory for the treatment of heart failure,and comprehensively analyzed the core prescription,used the network pharmacological method to obtain the target of the core prescription and the biological process and pathway of participation in the treatment of heart failure,and discussed the potential mechanism of the core prescription for the treatment of heart failure,providing a scientific basis for traditional Chinese medicine to treat this disease.Materials and methods: Collect the relevant data of patients with heart failure diagnosed and treated by Professor Zhang Mingxue in the inpatient department of the Affiliated Hospital of Liaoning University of Traditional Chinese Medicine from January 1,2018 to January 31,2022,include a total of 268 cases according to the scheduling standard,and extract relevant data to establish a database;use Excel,The traditional Chinese medicine inheritance auxiliary platform and SPSS Statistics 21 carry out descriptive statistical analysis,association rule analysis and cluster analysis of prescription data.According to the above analysis results combined with clinical practice,it further summarizes and summarizes Professor Zhang Mingxue’s prescription drug use rules and academic ideas for the treatment of heart failure,and comprehensively analyzes it.The drug composition of the core prescription.Screen the active ingredients and target information of the core prescription through the traditional Chinese medicine system pharmacology database and analysis platform TCMSP database,obtain the gene name corresponding to the target through the Uniprot database,and uniformly standardize the protein target name;use Gene Cards and Dis Ge NE T disease gene database retrieves and obtains gene targets related to heart failure,uses the Venny2.1 platform to intersect drug active ingredients and disease targets,obtains the intersection target of drug diseases,and draws a Wayne diagram;builds a protein interaction network(PPI)model through the String platform.Type;use the Cytoscape 3.9.1 software to draw the obtained intersection target into a "Chinese medicine-component-target" network diagram,and organize and modify the protein interaction network model;use the Metascape platform to carry out the GO function enrichment analysis of the protein target and KEGG pathway enrichment.The analysis is analyzed,and the analysis results are visualized through the bubble diagram drawn by the Weishengxin platform,and then the potential mechanism of the core prescription treatment of heart failure is discussed.Results:1.statistics of general case: In the sex statistics of 268 patients,female patients account for61.94%,more than male patients;the age distribution is mainly concentrated in elderly patients,accounting for 52.24%.2.Statistics of TCM syndrome: 268 patients have the highest frequency of qi-deficiency(97.76%)in pathological syndrome,followed by phlegm-turbidity(80.6%),retained-fluid(62.69%),blood-stasis(59.33%),qi-stagnation(37.69%),yang-deficiency(32.46%),yin-deficiency(16.04%),heat-toxic(7.09%).3.Statistics on drug use: 268 prescriptions contain 146 traditional Chinese medicines,of which 19 medicines are used more than 90 times,in order of Trichosanthes kirilowii Maxim,Pinellia ternata,astragalus,Longstamen Onion Bulb,American ginseng,Root of Chinese Thorowax,Lisianthus,Cimicifugae Rhizoma,Fuling,Atractylodes,Zexie,Red Paeony Root,Fossilizid,Peach Seed,Safflower,Spina Date Seed,Yuanzhi,Lesser Galangal Rhizome,Licorice;the most frequently used in the four characters is warming;the most frequently used in the five tastes is sweeting;the medicine belongs to the lung meridians,spleen meridians,stomach meridians,liver meridians,and heart meridians;the types of drugs are tonifying deficiency drugs,uplifting drugs.4.Association rules and cluster analysis: The second-order drug combinations with high support in the association rules include Pinellia ternata and Longstamen Onion Bulb,Trichosanthes kirilowii Maxim and Longstamen Onion Bulb,astragalus and American ginseng.Further through cluster analysis,19 traditional Chinese medicine with a frequency of≥ 90 times is selected,and a total of 7 types of drug combinations and drug pairs are obtained:Category 1: Root of Chinese Thorowax,Lisianthus,Cimicifugae Rhizoma;Category 2: Pinellia ternata,Longstamen Onion Bulb,Trichosanthes kirilowii Maxim,astragalus,American ginseng;Category 3:Fuling,Atractylodes,Zexie;Category 4: Fossilizid,licorice;Category 5:Peach Seed,safflower,Red Paeony Root;Category 6:Spina Date Seed,Yuanzhi;Category7:Galangal Rhizome.Based on the results of data mining and the clinical experience of the tutor,the core prescription names Sheng Qi Gua Lou Xie Bai Ban Xia(Trichosanthes kirilowii Maxim,Pinellia ternata,astragalus,Longstamen Onion Bulb,American ginseng,Root of Chinese Thorowax,Lisianthus,Cimicifugae Rhizoma).5.96 active ingredients of the core prescription were obtained,including quercetin,Stigmasterol,beta-sitosterol,isorhamnetin,kaempferol,etc.as the main active ingredients;there are 96 targets for the intersection of drugs and diseases,including IL6,VEGFA,AKT1,MMP9,PTGS2,IL1 B,CCL CXCL8,CASP3,IL10,TNF,IFNG,etc.are the keytargets;GO functional enrichment analysis results mainly involve membrane raft,external side of plasma membrane,extracellular matrix,endoplasmic reticulum lumen,vesicle lumen,response to nutrient levels,response to oxygen levels,response to inorganic substance,responseto peptide,cellular response to lipid,cytokine activity,transcription coregulator binding,protein homodimerization activity,protease binding;KEGG pathway enrichment analysis shows that genes are mainly enriched in Lipid and atherosclerosis,Pathways in cancer,Proteoglycans in cancer,IL-17 signaling pathway,Human cytomegalovirus infection,Relaxin signaling pathway,etc.Conclusion:1.Data mining research shows that the heart failure is located in the heart and is closely related to the lungs.The pathogenesis of Heart failure is always put deficiency first,with qi-deficiency as the cause of the disease throughout,phlegm-turbidity,blood-stasis,and retained-fluid as the superficial syndrome,mutual cause and effect.The tutor takes qi,phlegm,blood circulation,and fluid as the basic treatment methods,supplemented by calming the mind,calming the liver,clearing heat and other methods.The core prescriptions of the tutor’s clinical treatment of heart failure are composed of Trichosanthes kirilowii Maxim,Pinellia ternata,astragalus,Longstamen Onion Bulb,American ginseng,Root of Chinese Thorowax,Lisianthus,Cimicifugae Rhizoma.According to qi Lifting medicine,phlegm expelling medicine,blood circulation medicine,fluid draining medicine,warming medicine,etc.,each prescription can be given the characteristics of orderly lifting qi,keeping yin and yang in balance.2.Network pharmacological research shows that the core prescription treatment for heart failure has the characteristics of multi-component,multi-target and multi-channel overall regulation.Its potential action mechanism may be affected by the main active ingredients such as quercetin,Stigmasterol,beta-sitosterol,isorhamnetin,Kaempferol,etc.The expression of key genetic targets such as IL6、VEGFA、AKT1、MMP9、PTGS2、IL1B regulates Lipid and atherosclerosis,IL-17 signaling pathway,Relaxin signaling pathway,etc.,produces inhibition of overexpression of inflammatory factors,reduces oxidative stress,protects vascular endothelial and myocardial cells,inhibits ventricular remodeling and good for the blood oxygen supply of the heart. |
