Objective:To understand the current situation of psychological distress in COP D patients,to analyze the relationship between the physiology,self-management ab-ility,family support,and degree of illness of COPD patients and psychological distress,and to explore the influence of self-management mode on psychological distress in COPD patients,with the aim of improving the quality of life of COP D patients,improving lung function and patients’ symptoms and prognosis,reducing the number of repeated hospitalizations and shortening the length of hospitaliz-ation,and reducing family and The present study was conducted to analyze the interrelationship between these factors and to provide a basis for the development of specialized self-management treatment plans for patients.Methods:This study is a cross-sectional study.Using convenience sampling met-hod,150 COPD patients who were hospitalized in the Respiratory Department of a tertiary A hospital in Liaoning Province were selected from January to December 2022.COPD patients were surveyed using the General Information Question-naire,the Heart Distress Scale,the Modified Medical Research Council Respiratory Distress Scale(m MRC)in the United Kingdom for analysis,and the COPD Patient Self-Management Scale.SPSS 25.0 software was used to process the data.Counting data was represented by[n(%)],and measurement data was represented by((?)±s).Hypothesis tests were conducted using t-tests and analysis of variance.Multivariate analysis was conducted using logistic regression analysis,and P<0.05 was considered statistically significant.Results:1.150 COPD patients were screened by using DT,and the results showed that psychological distress was at a moderate level,and the mean value of psychological distress thermometer score was(5.59±2.10),among which significant psychologic-al distress 127 patients were detected,and the positive detection rate of psychological distress was 84.6%.2.The self-management score of COPD patients was(147.13±24.80),which was at a moderate level.The results are sorted from high to low according to the scor-es of each dimension:daily life management(43.08±7.71),emotion management(32.64±6.55),symptom management(25.62±4.63),information management(23.42±5.19),self-efficacy(22.37±5.79).3.The mean value of the Modified Medical Research Council Dyspnea Scale score was(2.56±1.16),with mild dyspnea accounting for 18%,moderate dyspnea accounting for 39.3%,and severe dyspnea accounting for 42.7%.There is a positive cor-relation between m MRC score and psychological distress in COPD patients(r=0.541,P<0.01).4.Significant psychological distress was significantly correlated with self-manage ment in COPD patients,with psychological distress score being associated with symptom management score(r=-0.174,P<0.05),daily life management score(r=-0.567,P<0.01),emotion management score(r=-0.599,P<0.01),information management score(r=-0.496,P<0.01)were highly negatively correlated with self-efficacy score s(r=-0.605,P<0.01).5.The results of influencing factor analysis showed that univariate analysis showed statistically significant differences in educational background,monthly income,course of disease,number of exacerbations of symptoms in the past year,COPD grading,and psychological pain scores of COPD patients(P<0.05).The psychological distress of COPD patients is positively correlated with the degree of difficul-ty breathing,and negatively correlated with self-management ability.The course of the disease,the number of acute exacerbations of symptoms in the past year,COPD grading,education level,income,m MRC,symptom management,daily life man-agement,emotional management,information management,self-efficacy,and the total score of the first five dimensions were included as independent variables in logistic regression analysis,the results showed that the number of exacerbations of symptoms in the past year(P=0.016,β=1.318),educational background(P=0.026,β=-1895),and emotional management(P=0.014,β=-0.161)were significant influencing fa-ctors for the occurrence of psychological distress in COPD patients.Conclusion:1.The psychological distress of COPD patients is at a moderate level,and the det-ection rate of significant psychological pain is relatively high.In clinical work,the psychological state of COPD patients should be seriously affected visual.2.The main issues that cause psychological pain in COPD patients are economy,worry,and sleep.3.The occurrence of psychological distress in COPD patients is positively correla-ted with various grades of m MRC,and negatively correlated with various dimens-ions of self-management ability.4.The key factors leading to significant psychological distress in COPD patients are the number of acute exacerbations of symptoms in the past year(hospitalizati-on),educational background,and emotional management.
