Objectives To explore the status quo and influencing factors of psychological distress in stroke patients,to improving the mental health level of stroke patients and establish relevant continuous nursing measures in clinical nursing.Methods During November 2019 to June 2020,Using the convenience sampling method,a total of 716 stroke patients from hospitalized in the Department of Neurology,Affiliated Hospital of North China University of science and technology of tangshan city,hebei province,were questionnaire investigation,including general situation questionnaire(demographic characteristic: age,gender,marital status,education level,et al;disease related information: course of disease,number of stroke,number of merged chronic disease,et al),Distress Thermometer,Stroke Self-efficacy Questionnaire,Medical Coping Modes Questionnaire,Brief Illness Perception Questionnaire,Functional Independence Measure,NIH Stroke Scale,The Hospital Anxiety and Depression Scale.Using Excel and SPSS22.0 statistical software to analyze the data.Descriptive statistical analysis was used to analyze the general situation of stroke patients.T test and analysis of variance were used to compare the differences of psychological distress of stroke patients in different demographic data,list of problems,self-efficacy,social support,NIHSS and hospital anxiety and depression.Pearson correlation analysis was used to analyze the relationship between coping style,disease perception,functional independence and psychological distress.Multiple stepwise regression analysis was used to explore the influencing factors of psychological distress.Results 1 The status of psychological distress of stroke patients: the score of psychological distress of stroke patients was(5.05±2.44),there were 196 stroke patients with mild psychological distress,accounting for 27.4% of the total sample,with a score of(2.23±0.98),were 328 patients(45.4%)with moderate psychological distress,and the score was(4.82±0.82),were 192 patients(26.6%)with severe psychological distress and the score was(8.32±1.08).2 Single factor analysis results of influencing factors of the psychological distress in the stroke patients:gender,age,education level,residence,personal monthly income,type of medical insurance,smoking history,course of disease,number of stroke,history of hypertension,history of coronary heart disease,number of chronic diseases,informed status,transportation,economic status,family responsibility,appearance,communication,dysuria,constipation,fatigue,activities(such as walking),pain,sleep,swallowing,getting along with family,anger,depression,fear,tension,sadness,worry,loss of interest in daily activities,self blame,self-efficacy,social support,NIHSS,hospital anxiety,hospital depression affect the psychological pain of stroke patients,the difference was statistically significant(P<0.05).3 It is suggested by correlation analysis that stroke patients’ avoidance and surrender coping was positively correlated with their the total score of psychological distress,while face coping was negatively correlated with their the total score of psychological distress,the difference was statistically significant(P<0.05);Stroke patients’ disease perception was positively correlated with their the total score of psychological distress(P<0.05);Stroke patients’ motor function and cognitive function was negatively correlated with their the total score of psychological distress the difference was statistically significant(P<0.05).4 Multivariate stepwise regression analysis of influencing factors of the psychological distress in the stroke patients: selfefficacy,NIHSS,social support,age,smoking history,functional independence measure,hospital depression,personal monthly income,transportation,swallowing,disease perception,sleep,loss of interest in daily activities enters the regression equation(P<0.05),those factors could explain 82.3% of total variance in the psychological distress.That was the stroke patients with low self-efficacy,severe stroke injury,low social support,under 59 years old,smoking history,low functional independence evaluation,severe hospital depression,low personal monthly income,travel difficulties,dysphagia,high disease perception,poor sleep,loss of interest in daily activities had more severe psychological distress;Conclusions 1 The psychological distress of the stroke patients was middle level.2 The main influence factors of the stroke patients were self-efficacy,NIHSS,social support,age,smoking history,functional independence measure,hospital depression,personal monthly income,transportation,swallowing,disease perception,sleep,loss of interest in daily activities.Figure 0;Table 23;Reference 152...
