Purpose : To investigate the effect of Anti-Vascular Targeting Drugs attenuated by Traditional Chinese Medicine on advanced solid tumors and to analysis of TCM syndrome differentiation and drug use of Anlotinib in advanced lung cancer.Material and method:A retrospective case study was used to screen the patients who were treated with Anti-Vascular Targeted Drugs from January,2020 to December,2022 in the Department of Oncology,Affiliated Hospital of Liaoning University of Traditional Chinese Medicine;Collect the general situation of cases, treatment process, four diagnostic data of traditional Chinese medicine, traditional Chinese medicine and other information to establish a database. The counting data is described in the number of cases and percentage.Chi-square test is used for inter-group comparison.COX multifactor analysis is used to explore the factors that affect the progression-free survival(PFS).SPSS software is used to cluster analysis of the four diagnostic methods of traditional Chinese medicine included in the study.Determine a reasonable TCM syndrome type based on clinical practice,TCM internal medicine and TCM syndrome differentiation of Lin Hongsheng’s lung cancer.Frequency statistics and classification of the Chinese herbal decoction of the included patients are carried out to explore the law of medication.Log-rank test(Graph Pad Prism)were used for statistical analysis of the receipt survival curve, and p<.05 was significant.Results:1.Multi-factor analysis: the overall Cox analysis results showed that the overall difference of PFS was statistically significant(p=0.028);Among them, gene mutation target,Chinese medicine addition, metastasis, and KPS score before treatment are the main risk factors affecting prognosis.2.The short-term efficacy of Chinese medicine combined with Anti-Vascular Targeted Drugs in the treatment of advanced solid tumors: The DCR of Chinese medicines section was higher than that of control group(p= 0.034).Long-term efficacy: The mean and median PFS of the Chinese medicines section were both longer than those of the control group, and the differences were remarkable after testing. The mean and median survival(OS)of the Chinese medicines section was longer than that of the control group, and non-significant difference was found by Log-rank test.3.Side effects: During the observation,10 side effects related to Anti-Vascular Targeted Drugs were found, including fatigue, hypertension, proteinuria, etc.Bleeding events in Chinese medicines section(14.13%)was higher than that in control group(13.46%),the incidence of other adverse events in observation group was lower than that in control group, and the incidence of fatigue and side effects of skin and subcutaneous tissue in Chinese medicines section was lower than that in control group(p< 0.05).To treat fatigue, many traditional Chinese medicines are used, such as Radix Pseudostellariae,Radix Astragali,Rhizoma Atractylodis Macrocephalae and Rhizoma Dioscoreae.Traditional Chinese medicines with heat-clearing and detoxicating functions are often used to treat rashes, such as Sophora flavescens,Cortex Dictamni Radicis,Flos Lonicerae,Kochia scoparia,Periostracum Cicadae,etc.4.The TCM syndrome types of patients with advanced lung cancer using antirotinib can be divided into Yin deficiency and toxic heat(37.5%),Qi and Yin deficiency(22.5%),blood stasis obstructing lung collaterals(22.5%),phlegm-dampness accumulation of lung(17.5%).Conditions of TCM decoction pieces: The most frequently used TCM decoction pieces were processed licorice,Radix pseudostellariae,Radix pseudostellariae, poria cocos, white Rhizome,Radix pseudostellariae,Fritillaria thunbergii,Radix gallicae gallicae,Coix seed, oyster, almond,Schistoderma schistoderma and Hedyodylla.According to the high-frequency symptoms, they can be divided into 9 categories, such as relieving cough, relieving phlegm and relieving asthma, supplementing and clearing heat.Conclusion:1.COX multi-factor analysis results showed that patients with good condition, gene mutation target,Chinese medicine application, and no distant metastasis before treatment could significantly prolong the PFS of patients with advanced solid tumors treated with Anti-Vascular Targeted Drugs.2.Log-rank test showed that the traditional Chinese medicine decoction could significantly prolong the PFS of patients with Anti-Vascular Targeted Drugs;TCM treatment based on syndrome differentiation can significantly prolong the use of PFS in patients with advanced lung cancer.3.Traditional Chinese medicine decoction can significantly improve the clinical benefit rate of patients who use Anti-Vascular Targeted Drugs.4.Traditional Chinese medicine decoction can reduce the incidence of side effects related to Anti-Vascular Targeted Drugs, especially the effects of traditional Chinese medicine on fatigue, skin and subcutaneous tissue.5.TCM syndrome differentiation: the TCM syndrome of patients with advanced lung cancer who use Anlotinib is mainly yin deficiency and toxic heat.6.Drug use analysis: The results of antirotinib in the treatment of advanced lung cancer patients showed that supplementing qi,tonifying deficiency and dispelling dampness were the most frequently used.Tonifying, relieving cough and phlegm, relieving asthma and clearing heat are the most widely used drugs. |
