Objective: To analyze the correlation between the number,length,and invasiveness of spicule signs in malignant lung tumors,and evaluate the feasibility of judging the invasiveness of lung cancer based on the details of spicule signs.Methods: Retrospective analysis of cases of lung cancer confirmed by pathology after chest CT examination at the Second Affiliated Hospital of Xingtai Medical College from March 1,2020 to June 30,2022,classified by the number and length of spicules.A Spicule count of ≥ 5 was assigned to the study group,while a Spicule count of <5 was assigned to the control group.A Spicule length of ≥ 5mm is considered a long Spicule group,and<5mm is considered a short Spicule group.Analyze the correlation between indicators such as the presence,length,and quantity of spicule lesions in chest CT between different groups and tumor invasiveness,such as the longest diameter of the tumor,Ki67 index,and distant metastasis.Analyze the relationship between TNM staging and the number and length of Spicules for some surgical cases.The obtained data was subjected to statistical analysis using SPSS 25.0 software,with P<0.05 indicating statistically significant differences.Results:1.When the number of spicules is ≥ 5,there are differences in tumor Ki67 index,pathological differentiation degree,and the presence of distant metastasis,with statistical significance(P<0.05).When the number of spicules is ≥ 5,the tumor is more invasive.2.When the spicules length is ≥ 5mm,there is a difference in the invasive indicators of the tumor compared to those with Spicule length<5mm,and the difference is statistically significant(P<0.05).When the spicules length is ≥ 5mm,the tumor’s invasiveness is stronger.3.The later the TNM staging of surgical patients,the more spicules they have on their chest CT,and the longer the length of the longest spicules.Conclusions: There is a certain correlation between the detailed features of chest CT spiculation signs and tumor invasiveness in malignant lung tumors.When the number of spicules on chest CT of pulmonary malignant tumors is ≥ 5 or the longest spicule length is ≥ 5mm,the tumor shows longer primary tumor diameter,higher Ki67 index,poor differentiation,often accompanied by distant metastasis,and later TNM staging,suggesting that the tumor may be more invasive.
