Efficacy Observation Of Intense Pulsed Light And Q-switched 1064nm Nd:YAG Laser In Treating Post-inflammatory Hyperpigmentation In Acne: A Split-face Study

Objective : To evaluate the efficacy,safety,and superiority of intense pulsed light and Q-switched 1064 nm Nd:YAG laser in treating post-inflammatory hyperpigmentation in acne using a split-face mode.Methods: Twenty-five patients with facial post-inflammatory hyperpigmentation in acne were included in this study and sorted in order from 1 to 25 according to the time of recruitment.The patients were randomly divided into two groups,A and B,by a random number method.Group A received intense pulsed light treatment on the left side of the face and Q-switched 1064 nm Nd:YAG laser treatment on the right side,and Group B received Q-switched 1064 nm Nd:YAG laser treatment on the left side and intense pulsed light treatment on the right side.The treatment was performed three times with a two-week interval.Before each treatment and two weeks after the final treatment,LAB,CK,and VISIA were examined to evaluate the inter-group and intra-group comparisons at the same time point on both sides.The severity of post-inflammatory hyperpigmentation and erythema were evaluated at baseline and two weeks after the final treatment.Adverse reactions and pain visual analogue scale were recorded after each treatment,and patient satisfaction was assessed two weeks after the final treatment.Results: This study completed a total of 23 subjects’ treatment,evaluation,and follow-up according to the plan.Objective data: LAB,CK,VISIA and other data conform to normal distribution.By conducting repeated-measures analysis of variance and paired t-test analysis between groups,it was found that indicators related to post-acne pigmentation such as melanin value,brown spots,patches,UV spots,and L value were improved.Moreover,the efficacy of Q-switched 1064 nm Nd:YAG laser in treating post-acne pigmentation was superior to intense pulsed light,and the difference was statistically significant.On the other hand,indicators related to post-acne erythema such as erythema value,red value,and A value were also improved,and the efficacy of intense pulsed light in treating post-acne erythema was superior to Q-switched 1064 nm Nd:YAG laser,and the difference was statistically significant.Although the B value suggested some degree of improvement with both treatment methods in single-factor analysis of variance,paired-sample t-test analysis between the two groups showed no statistical difference in efficacy.Subjective data:Four-point erythema severity scale and pigmentation severity scale were used to evaluate the severity of post-acne inflammatory pigmentation before and after treatment on the cheeks.The Wilcoxon signed-rank test showed that the differences in erythema severity and pigmentation severity before and after treatment with intense pulsed light were statistically significant,and the differences in erythema severity and pigmentation severity before and after treatment with Q-switched 1064 nm Nd:YAG laser were also statistically significant,further confirming that both intense pulsed light and Q-switched 1064 nm Nd:YAG laser can improve post-acne erythema and post-acne pigmentation.The satisfaction survey showed no significant difference in satisfaction scores between the two treatment methods.The visual analogue scale score for pain showed that the pain score on the Q-switched 1064 nm Nd:YAG laser side was lower than that on the intense pulsed light side,and the difference was statistically significant.Adverse reactions: Both intense pulsed light and Q-switched1064 nm Nd:YAG laser treatments resulted in mild erythema and swelling after surgery,they disappeared after icing for 30 minutes.Conclusion: The application of intense pulsed light and Q-switched 1064 nm Nd:YAG laser in the treatment of post-inflammatory hyperpigmentation in acne patients can effectively improve both post-acne erythema and post-inflammatory hyperpigmentation.The treatment methods are safe and effective,with intense pulsed light showing better efficacy in treating post-acne erythema than Q-switched 1064 nm Nd:YAG laser,and Q-switched 1064 nm Nd:YAG laser showing better efficacy in treating post-inflammatory hyperpigmentation than intense pulsed light.
