Mr.Haibiao Lai is a professor of Guangzhou University of Chinese Medicine,a famous Chinese medicine practitioner in Guangdong Province,and the academic leader of National Key Clinical Specialties(Nephrology).He has been engaged in clinical practice in Chinese medicine for 30 years,specializing in treating difficult and miscellaneous diseases with classical prescriptions,and has comprehensive research and rich clinical experience in urological diseases such as urolithiasis,prostate diseases and male diseases.By following the teacher for consultation and collecting medical cases related to urolithiasis,Professor Lai’s prescription pattern in the treatment of urolithiasis is researched in this project to provide new ideas for clinical practice.ObjectiveThe prescription pattern of Professor Haibiao Lai ’s treatment of urolithiasis was analyzed based on data mining technology,and potential new prescriptions were deeply explored to provide young clinicians with prescription experience.MtehodsIn this research,cases of urolithiasis treated by Professor Lai Haibiao from January 1,2019 to December 31,2022 were collected,which were then strictly screened according to the inclusion and exclusion criteria,recorded and organized by using the "Traditional Chinese Medicine Inheritance Support System Software V2.5".An information database of Professor Haibiao Lai’s medical cases was constructed,the patterns of Professor Haibiao Lai’s prescription in the treatment of urolithiasis were analyzed and explored by using the frequency analysis,correlation analysis,and cluster analysis of the software.RusultsA total of 174 first consultation cases of urolithiasis patients were included in this project in its entirety and analyzed through the “Traditional Chinese Medicine Inheritance Support platform”,and the number of male patients with urolithiasis was significantly higher than that of female patients.The mean age of onset was 49.74±14.35 years,with a peak of 30-60 years,indicating that stone formation is influenced by gender and age.The core pathogenesis of urolithiasis in the study population was damp invasion of lower energizer,kidney Qi deficiency,and Qi stagnation & blood stasis.A total of 93 drugs were involved,and the top eight drugs in terms of frequency of use were endothelium corneum gigeriae galli,lysimachia,lygodium japonicum,achyranthes,radix curcumae,saponaria vaccaria,eucommia,and white peony.The four natures of drugs with the highest using frequency were mostly cold,neutral and warm,with cold being the main characteristic.The five flavors of drugs with the highest using frequency were mostly sweet,bitter and salty,with sweetness as the main taste.The meridian channel tropism of drugs with the highest using frequency were bladder channel and liver channel.Based on association rules,seven core drug flavors were obtained,including endothelium corneum gigeriae galli,lysimachia,lygodium japonicum,achyranthes,radix curcumae,Saponaria vaccaria,and eucommia.Based on the complex system entropy cluster analysis,six core drug combinations were obtained,which were saponaria vaccaria-liquorice-achyranthes,saponaria vaccariaachyranthes-radix bupleuri,endothelium corneum gigeriae galli-cogongrass rhizomelysimachia-hairyvein agrimony,endothelium corneum gigeriae galli-cogongrass rhizome-lygodium japonicum-lysimachia-radix curcumae,poria cocos-lygodium japonicum-alisma orientale juzep-lysimachia-radix curcumae,endothelium corneum gigeriae galli-poria cocos-achyranthes-lygodium japonicum-lysimachia-radix curcumae.Based on unsupervised entropy hierarchical cluster analysis,three potential new prescriptions were obtained,which were saponaria vaccaria-liquorice-achyranthes-radix bupleuri,endothelium corneum gigeriae galli-cogongrass rhizome-lysimachiahairyvein agrimony-lygodium japonicum-radix curcumae,poria cocos-lygodium japonicum-alisma orientale juzep-lysimachia-radix curcumae-endothelium corneum gigeriae galli-achyranthes.ConclusionBased on data mining,it is found that Professor Haibiao Lai’s treatment of urolithiasis is guided by the theory of "taking kidney asthenia as the root cause,bladder damp-heat as the symptom,and Qi stagnation & blood stasis as the pathological changes",and the treatment at the time of clinical symptoms is based on removing heat and dampness,relieving stranguria and discharge of calculus,promoting Qi to activate blood and removing blood stasis,as well as tonifying kidney Qi.Commonly used drugs are mainly cold,sweet and neutral in nature.The main drugs used are endothelium corneum gigeriae galli,lysimachia,lygodium japonicum,achyranthes,radix curcumae,saponaria vaccaria,eucommia,and white peony,and the treatment is added and subtracted according to their concurrent symptoms.
