ObjectiveTo investigate the clinical efficacy of the scalp acupuncture plus medicine umbilical moxibustion in diarrhea-predominant irritable bowel syndrome(IBS-D)patients with liver-stagnation and spleen-deficiency.By observing the statistical changes of IBS-SSS scale score,IBS-DSQ score,frequancy of defecation,TCM syndrome score of liver-stagnation and spleen-deficiency,abnormal electrical response activity parameters of the colon to compare the treatment effect of " Scalp Acupuncture plus Medicine Umbilical Moxibustion " and "conventional acupuncture" in the treatment of diarrhea-predominant Irritable Bowel Syndrome.Providing objective clinical evidence proving the advantages and efficacy of "Guan" scalp acupuncture plus medicine umbilical moxibustion in the treatment of this disease.MethodsIn this study,80 patients with IBS-D who met the inclusion criteria and recruited from social and Sanya Hospital of Traditional Chinese Medicine affiliated to Guangzhou University of TCM from January 2022 to January 2023 were allocated into test group(n=40)and control group(n=40)by the random number table method.The control group was given conventional acupuncture therapy;the test group was given scalp acupuncture plus medicine umbilical moxibustion.Both groups were applied with treatment 5 times a week for 4 weeks.The IBS-SSS scale score,IBS-DSQ score,frequancy of defecation,TCM syndrome score of liver-stagnation and spleen-deficiency,and abnormal electrical response activity parameters of the colon were used to compare the clinical conditions of the two groups before and after treatment,and finally analyze and compare the clinical efficacy of the two groups.Statistical analysis was performed by IBM SPSS Statistics 26.ResultsA total of 80 patients were included in this study,one sample was dropped out from the control group and one was excluded,meanwhile one was dropped out from the test group,resulting in a total of 77 samples included in the statistical analysis,including 38 cases in the control group and 39 cases in the test group.1.Baseline data: no statistical differences were found between the two groups in terms of age,gender,disease duration,BMI,severity level of bowel syndrome,abnormal level of stool morphology,and the degree of bowel dysfunction(P>0.05),which were comparable.2.IBS-SSS score: There was no significant difference in IBS-SSS score and severity level of intestinal symptoms between the two groups before treatment(P>0.05),which were comparable.After treatment,the IBS-SSS scores in both groups were lower than those before treatment,and the difference was significant(P<0.01).When compared between groups after treatment,the reduction in IBS-SSS score in the test group was more significant than that in the control group(P<0.01).3.IBS-DSQ score: The differences in IBS-DSQ score and level of of abnormal stool morphology between the two groups before treatment were not statistically significant(P>0.05)and were comparable.After treatment,the IBS-DSQ scores in both groups were lower than those before treatment,and the difference was significant(P<0.01).In comparison between groups after treatment,the reduction in IBS-DSQ score was more significant in the test group compared with the control group(P<0.01).4.Frequancy of defecation: The differences between the two groups in terms of total number of defecation in the week before treatment and the maximum number of defecation in a single day was not statistically significant(P>0.05)and was comparable.After treatment,the total number of bowel defecation in a week and the maximum number of defecation in a single day in both groups were reduced significantly compared with those before treatment(P<0.01).When compared between groups after treatment,the reduction of weekly cumulative defecation number and the daily maximum number of defecation in the test group was more significant than that in the control group(P<0.05).5.Abnormal electrical response activity ratio in colon: There was no significant difference in the rate of abnormal electrical response activity in colon between the two groups before treatment(P>0.05),which were comparable.After treatment,the rate of abnormal electrical response activity in both groups was lower than that before treatment,and the difference was significant(P<0.01).In comparison between groups after treatment,the the rate of abnormal electrical response activity in the test group was reduced more significantly than that in the control group(P<0.01).6.TCM syndrome points: The difference in TCM syndrome points of liver-stagnation and spleen-deficiency between the two groups before treatment was not statistically significant(P>0.05)and was comparable.After treatment,the TCM points of both groups were lower than those before treatment,and the difference was significant(P<0.01).When compared between groups after treatment,the reduction in TCM points in the test group was more significant than that in the control group(P<0.01).7.Safety evaluation: No significant adverse reactions were found in both groups during treatment,and no significant abnormalities were found in all routine test before and after treatment,and the treatment method could be evaluated as safe.8.Comparison of efficacy: The total effective rates of the test group and the control group were 89.74% and 65.79% respectively,and the efficacy of the test group was significantly better than that of the control group(P<0.05).Conclusion1.Both scalp acupuncture plus medicine umbilical moxibustion and conventional acupuncture are effective in treating diarrhea-predominant irritable bowel syndrome;2.Scalp acupuncture plus medicine umbilical moxibustion is superior to conventional acupuncture in relieving bowel syndrome and relieving IBS-D TCM symptoms.3.The curative effect of scalp acupuncture plus medicine umbilical moxibustion in treating IBS-D is obviously better than that conventional acupuncture and moxibustion.
