| Objective: In the process of diagnosis and treatment of chronic diseases in the elderly,due to the poor physical function of the elderly and the pathological characteristics of chronic diseases for a long time,the elderly patients with chronic diseases need long-term drug treatment to control the disease or delay the development of the disease,and often need to use drugs in the treatment process.A combination of drugs to treat the disease.Existing research shows that the elderly often have drug-related problems in the treatment process that affect the treatment effect,and most of the drug-related problems are related to the lack of drug information transmission.Therefore,it is very important to ensure the continuity of medication information in the diagnosis and treatment of chronic diseases in the elderly.In this study,by reviewing and sorting out relevant literature at home and abroad,we constructed an evaluation index system for the continuity of medication information,in order to conduct a comprehensive assessment of the status quo of medication information continuity in elderly patients with chronic diseases.Methods:This research will sort out and summarize the relevant research status at home and abroad,find out the problem of information continuity,and screen out relevant evaluation indicators;sort out and classify the indicators from three dimensions,and initially construct an evaluation framework for information continuity;through the Delphi method,Carry out scientific evaluation dimension analysis,and revise and improve the index system according to the opinions of experts;score the importance of the indicators through expert consultation,and combine the analytic hierarchy process to give weights to each index to form a complete information continuity evaluation index system;Build the results for discussion,get inspiration and suggestions.Results:The evaluation index system initially constructed through literature review in this study includes 5 first-level indicators and 45 second-level indicators;14 experts in related fields were selected for two rounds of expert consultation,and all responses were received.The evaluation index system was revised twice by the Delphi method,and finally formed an evaluation index system including 6 first-level indexes and 33 second-level indexes;the weight of each index was determined by the analytic hierarchy process,and 6first-level indexes were used.The weights are: normative(0.1711),completeness(0.1663),accessibility(0.1710),timeliness(0.1679),accuracy(0.1636),and degree of utilization(0.1603),and finally formed a medication for elderly patients with chronic diseases.Information continuity evaluation index system;the establishment result of evaluation index system is discussed.Conclusion:Starting from the three dimensions of structure,process and result,the constructed evaluation system can further clarify the connotation of information continuity,and help improve the level of information continuity between medical institutions and elderly patients with chronic diseases.By discussing the results of the construction of the evaluation index system,it is found that the evaluation of the continuity of medication information should be paid enough attention,and strengthening the standardization of information is of great significance to the evaluation of information continuity. |
