| Objective:This study intends to investigate fungaus ball sinusitis(FBS)patients with Chronic Rhinosinusitis With Nasal polyps(CRSw NP)CT value,GOSS score,subjective and objective of local sinus bone in CRSw NP patients.To provide guidance for the diagnosis and treatment of fungal sinusitis and chronic sinusitis.Methods:According to the inclusion criteria,31 patients who underwent endoscopic sinus surgery for the first time at Qinhuangdao First Hospital from January2019 to December 2022 were selected as the FBS experimental group;32cases of chronic sinusitis with nasal polyps were included in the CRS experimental group;Cases of simple nasal septum deviation were the control group,20 cases.Among them,patients with fungal ball type nasosinusitis were all unilateral lesions,with the lesion side as the FBS experimental group and the opposite side as the FBS control group;According to the GOSS score,cases of fungal ball type sinusitis are divided into two groups: the osteitis group(osteitis grade ≥ 5 points)and the non osteitis group(osteitis grade<5 points);The cases of chronic sinusitis with nasal polyps and simple nasal septum deviation were divided into CRS osteitis group and CRS non osteitis group according to GOSS score.Results SPSS26.0 software was used to test the paired rank sum of the experimental group and the control group in the FBS group;For FRS group,CRS group and control group whose data conform to normal distribution,independent sample T test is conducted.For those who do not conform to normal distribution,Mann Whitney U test is used.All significance tests are bilateral tests,and P<0.05 is considered statistically significant.Results:1.In the cases of fungal bulb sinusitis,the CT values of FBS experimental group(31 cases)and control group(31 cases)were significantly different,and were positively correlated;1.1 In the cases of fungal bulb sinusitis,the CT values of FBS experimental group(31 cases)and control group(31 cases)were significantly different,and were positively correlated;1.2 There was a significant correlation between CT value and GOSS score in FBS osteitis group(r=0.689,P < 0.05).There was no significant correlation between CT value and Lund-Mackay score,SNOT-22 score and VAS score(r value was 0.05,0.402,0.046,P > 0.05);1.3 There was no significant correlation between Lund-Mackay score,SNOT-22 score,VAS score and GOSS score in FBS osteitis group(r =0.205,0.411,0.001,P > 0.05);2.The CT values of the experimental group(32 cases)and the control group(20 cases)were compared,and the CT values of the experimental group were higher than those of the control group and the difference was statistically significant(P < 0.05);2.1 In the CRS group,preoperative GOSS score,Lund-Mackay score,SNOT-22 score and VAS score were significantly different between osteitis group(25 cases)and non-osteitis group(20 cases)(P < 0.05);2.2 CT values in CRS osteitis group were significantly correlated with GOSS and Lund-Mackay scores(r = 0.542,0.554,P < 0.05).There was no significant correlation between CT value and SNOT-22 score and VAS score(r = 0.393 and 0.129,P > 0.05);2.3There was no significant correlation between Lund-Mackay score,SNOT-22 score,VAS score and GOSS score in CRS osteitis group(r =0.058,0.336,0.335,P > 0.05);3.The mean CT value of the CRS group was higher than that of the FBS group,and the difference was statistically significant(P < 0.05);Conclusion:1.Bone lesions were present in both patients with fungal ball sinusitis and chronic sinusitis with nasal polyps,and the CT value of chronic sinusitis with nasal polyps was higher than that of fungal ball sinusitis,suggesting more obvious signs of osteitis.2.CT values in fungal bulb sinusitis and chronic sinusitis with nasal polyps were well correlated with GOSS scores,suggesting that CT values could evaluate the severity of osteitis to a certain extent.3.The correlation between Lund-Mackay score,SNOT-22 score,VAS score and GOSS score was not clear in patients with chronic sinusitis. |
