Objective:To study the correlation between skin physiological indicators and disease severity and prognosis in patients with cosmetic contact dermatitis,and to provide a noninvasive detection method for the diagnosis and treatment of cosmetic contact dermatitis;Methods:A total of 120 patients with facial cosmetic contact dermatitis who visited the dermatology clinic of Guilin Medical College from June 2022 to December 2022 were randomly selected for a 2-week follow-up based on the inclusion criteria,exclusion criteria,and exclusion criteria.The main follow-up items were:clinical symptom scores,skin biochemical indicators(TEWL,skin keratin moisture content,skin erythema index),and Visia testing.The collected data were collated and statistically analyzed using SPSS 26.0.All selected subjects were approved by the Ethics Committee of Guilin Medical College.Results:(1)A total of 120 patients were included,including 11 males(9.17%)and 109 females(90.83%);Among 120 patients,87(72.50%)were younger than 40 years old,and 33(27.50%)were older than or equal to 40 years old;113 cases(94.17%)had a course of disease less than 1 week,and 7 cases(5.83%)had a course of disease greater than or equal to 1 week;73 cases(60.83%)had an incubation period of less than 72 hours,and 47 cases(39.17%)had an incubation period of more than 72 hours;58 cases(48.33%)used more than 7 types of cosmetics per day,and 62 cases(51.67%)used less than or equal to 7 types of cosmetics per day;23 cases(19.17%)used cosmetics more than 10times a day,97 cases(80.83%)used cosmetics less than or equal to 10 times a day;51 cases(42.5%)had anxiety about the disease and 69 cases(57.5%)had no anxiety about the disease;(2)The positive rate of patch test in 120 patients was 45.83%,of which 0(0.00%)were extremely positive(+++),1(0.83%)were strongly positive(++),54(45.00%)were weakly positive(+),6(±)were suspected positive(5.00%),and 59(-)were negative(49.17%);There were 32patients(26.67%)with positive patch tests in patients with an incubation period of less than 72 hours,and 23 patients(19.17%)with positive patch tests in patients with an incubation period of more than 72 hours;(3)According to the clinical symptom score,the effective rate of 120 patients after 1 week of treatment was 87.50%,and the effective rate after 2 weeks of treatment was93.33%.Chi-square test showed no significant difference(P=0.187);(4)After one week of treatment on the forehead of 120 patients,TEWL(P=0.016)and skin erythema index(P=0.000)were significantly improved compared to the baseline period;After one week of treatment on the cheek,TEWL(P=0.006)and skin erythema indicators were significantly improved compared to baseline(P=0.001);After 1 week of treatment on the nose,the skin erythema index(P=0.027)was significantly improved compared to the baseline period;After one week of treatment on the chin,TEWL(P=0.001)and skin erythema index(P=0.000)were significantly improved compared to the baseline period;After 2weeks of treatment on the forehead,TEWL(P=0.000),skin stratum corneum water content(P=0.002),and skin erythema index(P=0.000)were significantly improved compared to the baseline period;After 2 weeks of treatment on the cheek,TEWL(P=0.000),skin stratum corneum water content(P=0.004),and skin erythema index(P=0.000)were significantly improved compared to the baseline period;After 2 weeks of treatment for the nose,TEWL(P=0.004),skin keratin water content(P=0.018),and skin erythema index(P=0.000)were significantly improved compared to the baseline period;After 2 weeks of treatment on the chin,TEWL(P=0.000),skin stratum corneum water content(P=0.008),and skin erythema index(P=0.000)were significantly improved compared to the baseline period;(5)Through Spearman or point two column correlation analysis,TEWL(r_s=0.579),skin erythema index(r_s=0.501),anxiety(r=0.243),type of daily cosmetics used(r_s=0.458),and frequency of daily cosmetics used(r_s=0.461)in patients with cosmetic contact dermatitis were correlated with baseline disease severity(P<0.05),with a greater correlation coefficient for TEWL(r_s=0.579);(6)The TEWL(P=0.022),course of disease(P=0.006),type of daily cosmetics used(P=0.028),and frequency of daily cosmetics used(P=0.043)in patients with poor prognosis were significantly higher than those in patients with good prognosis.Conclusions:1.Skin physiological indicators such as TEWL and skin erythema index can reflect the severity of cosmetic contact dermatitis to a certain extent;2.TEWL has certain reference value for the prognosis of cosmetic contact dermatitis;3.The severity of cosmetic contact dermatitis is positively correlated with the patient’s anxiety,type of daily use of cosmetics,and frequency of daily use of cosmetics;4.The duration of the patient’s illness,anxiety,type of daily cosmetics use,and frequency of daily cosmetics use are independent prognostic factors;Doctors should pay attention to the psychological situation of patients with cosmetic contact dermatitis and the repair of skin barriers. |
