Objective: Atopic dermatitis(AD)is a chronic inflammatory skin disease characterized by severe itching and recurrent eczema-like skin lesions,which affects 20% of children and 3% of adults.The high prevalence and treatment cost make atopic dermatitis become the heaviest economic burden in the world.It is now clear that under the genetic background,skin barrier,immune system and bacterial flora interact with each other in the pathogenesis of atopic dermatitis.Skin barrier emulsion,topical corticosteroid or calcineurin inhibitor can control skin inflammation.However,the disease repeated and skin lesions itching are the difficulty in treatment.In order to clarify the mechanism of skin barrier repair emulsion in the treatment of atopic dermatitis and to find more effective methods to repair the skin barrier,this study studied the mechanism of skin barrier repair emulsion in the treatment of atopic dermatitis from four aspects: inflammation,pruritus,flora and metabolism.Methods: BALB/c mice ears were coated with topical calcipotriol for 14 days,resulting in atopic dermatitis.During the modeling process,skin barrier repairer was applied,and its effects on skin barrier,inflammation,flora and metabolism of mice were observed.HE sections were used to observe the epidermal thickness and lymphocyte infiltration in mouse ears,and the expressions of TSLP and IL-4 were detected by q PCR.Skin barrier repair was observed by detecting FLG gene expression,SC Hydration and skin TEWL.16s r RNA was used to detect the changes in the number of bacteria and fungi under the same skin area.Target metabolism and non-target metabolism were combined to detect the difference of metabolites in skin tissue between different groups.Results: Compared with calcipotriol group,the mouse ear epidermal thickness of the barrier repair emulsion group decreased(P< 0.0001),lymphocyte infiltration decreased,and the expression of TSLP and IL-4 decreased(P<0.05,NS).FLG expression increased(P<0.05),water content increased(P< 0.0001),and TEWL did not change significantly.The number of fungi in the same area of skin decreased(P<0.05),and the number of bacteria did not change significantly.Non-targeted metabolomics revealed up-regulation of phenylalanine,tyrosine and tryptophan metabolic pathways.Target metabolomics showed increased levels of inflammatory unsaturated fatty acid linoleic acid(P<0.05)and tryptophan(P<0.01).Conclusion: Topical using skin barrier repair emulsion can reduce the severity of atopic dermatitis,caused by calcipotriol in reducing inflammatory reaction,repairing skin barrier at skin lesion site,regulating microbial balance at skin lesion site,regulating skin tissue metabolism,and playing a important role in the treatment of atopic dermatitis.
