ObjectiveUnderstanding the current status of post-traumatic stress disorder,cognitive function,and quality of life in stroke patients,exploring the interrelationships between post-traumatic stress disorder,cognitive function,and quality of life,and further exploring whether cognitive function plays a mediating role between post-traumatic stress disorder and quality of life in stroke patients,in order to prevent and identify post-traumatic stress disorder in stroke patients,To provide theoretical basis for alleviating psychological disorders in stroke patients and improving their cognitive function and quality of life.Methods1.This study is a cross-sectional study using convenience sampling method,selecting322 eligible stroke patients from a tertiary A hospital in Shandong Province as the research subjects.The patients were investigated by self-designed general demography data questionnaire,post-traumatic stress disorder scale civilian version,Montreal cognitive assessment scale and stroke specific quality of life scale.2.SPSS25.0 software was used for data entry and statistical analysis of research data.Generally,demography and case data were analyzed by frequency,percentage or mean,and standard deviation.Posttraumatic stress disorder,cognitive function,and quality of life in stroke patients were described using mean and standard deviation.To explore the influencing factors of general Demography data on post-traumatic stress disorder,cognitive function and quality of life in stroke patients by one-way ANOVA.Exploring the relationship between post-traumatic stress disorder,cognitive function,and quality of life in stroke patients through Pearson correlation analysis.With PTSD as the independent variable,cognitive function as the intermediary variable,and quality of life as the dependent variable,process was used to analyze the mesomeric effect of cognitive function between PTSD and quality of life.Results1.The total score of post-traumatic stress disorder in stroke patients is(29.14±9.50),with 44 patients having a total score of 38 or higher,with a positive rate of 13.67%.The scores in each dimension range from high to low,including avoidance/numbness dimension(13.57 ± 4.87),high alertness dimension(8.14 ± 2.19),and re experience dimension(7.41±2.37);The total score of cognitive function was 24.00±4.22 points,with 108 out of 322 patients having a total score of less than 26 points.Patients with cognitive impairment accounted for 33.54%;The total score of quality of life is 177.32±42.71.2.The results of univariate analysis showed that there was a statistically significant difference in the scores of post-traumatic stress disorder among stroke patients in terms of age,occupation,education level,family income,and hospitalization payment method(P<0.05);There is a statistically significant difference in the scores of cognitive function among stroke patients in terms of age,occupation,and education leve(P<0.05)l;There were statistically significant differences in the scores of stroke patients’ quality of life in terms of occupation,education level,family income,hospitalization payment methods,and primary caregivers(P<0.05).3.Pearson correlation analysis showed a negative correlation between post-traumatic stress disorder and quality of life(r=-0.327,P<0.01);Cognitive function is positively correlated with quality of life(r=0.335,P<0.01);Post traumatic stress disorder is negatively correlated with cognitive function(r=-0.345,P<0.01).4.The results of multiple stepwise regression analysis showed that patients with post-traumatic stress disorder,cognitive function,annual family income,education level,payment methods for hospitalization expenses,and primary caregivers were the main influencing factors on the quality of life of stroke patients(p<0.05).These six variables can jointly explain 50.20% of the total variation in the quality of life level of stroke patients.5.The results of the process mediated model test showed that cognitive function partially mediated the relationship between post-traumatic stress disorder and quality of life.The total effect size value was-0.795(P<0.01),the direct effect was-0.690(P<0.01),and the indirect effect size value was-0.105(95% CI:-0.006~-0.001).The mesomeric effect accounted for 13.21% of the total effect.Conclusions1.The incidence of post-traumatic stress disorder in stroke patients is relatively high,with a low level of cognitive function and moderate quality of life.Posttraumatic stress disorder,cognitive function,annual family income,education level,payment methods for hospitalization expenses,and primary caregivers are the influencing factors of quality of life in stroke patients.There is a pairwise correlation between post-traumatic stress disorder,cognitive function,and quality of life in stroke patients.Cognitive function partially mediates the relationship between post-traumatic stress disorder and quality of life.Posttraumatic stress disorder can directly affect quality of life or indirectly affect it through cognitive function.2.The results of this study suggest that medical personnel should provide health education in the early stages of the disease,promote patients to adopt good coping styles to face the disease,correctly understand the risk of stroke,reduce patients’ psychological burden,prevent the occurrence of post-traumatic stress disorder,and thus improve patients’ cognitive function and quality of life.
